Effects of Gestation and Antenatal Steroid on Airway and Tissue Mechanics in Newborn Lambs

Abstract: The aim of this study was to partition airway and parenchymal mechanics in newborn lambs at different gestations and following variable exposure to antenatal maternal betamethasone using the forced oscillation technique (FOT). Pulmonary impedance data were collected in 37 sedated and intubated apneic lambs with the FOT between 0.5 and 20 Hz and fitted by a model to estimate airway resistance (Raw) and inertance (Iaw) and the coefficients of tissue resistance (GL) and elastance (HL). Total respiratory resistanc… Show more

“…ratory mechanics related to fetal lung maturation, as assessed in preterm lambs of differing gestations and/or exposure to maternal corticosteroid administration, are also most evident in the parameters of tissue resistance and tissue elastance (21). The observations are consistent with rapid development of the lung tissue occurring during the early neonatal period.…”

“…Flow during HFOV was measured using a hot-wire anemometer (Florian, Acutronic Medical Systems AG, Hirzel, Switzerland) placed between the distal end of the patient circuit and the TT. Partitioned lung mechanics were evaluated in the lambs at 1 h of age using a modification of the low-frequency forced oscillation technique as described previously (18). C ti was calculated from the coefficients of G and H determined from the forced oscillatory impedance data (19) as follows:…”

“…45,70,98,126 Of these factors, based on our animal data and human data from previous studies dealing with preterm gestations, it appears that the most effective strategy for accelerating reabsorption of fetal lung fluid would be the administration of exogenous glucocorticoids. 43,48,83,111 High doses of glucocorticoids have been shown to stimulate transcription of ENaC in several sodium transporting epithelia as well as in the lung. 126 In the alveolar epithelia, glucocorticoids were found to induce lung sodium reabsorption in the late gestation fetal lung.…”
