Effects of Levothyroxine Replacement Therapy on Parameters of Metabolic Syndrome and Atherosclerosis in Hypothyroid Patients: A Prospective Pilot Study

Abstract: The aim of this study was to investigate the effect of levothyroxine (LT4) replacement therapy during three months on some parameters of metabolic syndrome and atherosclerosis in patients with increased thyroid-stimulating hormone (TSH) level. This study included a group of 30 female patients with TSH level >4 mIU/L and 15 matched healthy controls. Intima media complex thickness (IMCT) and peak systolic flow velocity (PSFV) of superficial femoral artery were determined by Color Doppler scan. In hypothyroid sub… Show more

“…Sun et al reported that an increased level of lipoproteins and decreased level of HDL-C was associated with a higher incidence of cardiovascular disease (33). However, we did not find any significant effect of HBOT on the level of HDL-C, probably due to other parameters, such as age and diet (34,35). Another reason for not detecting a significant effect on HDL-C could lie in the small sample size and small effect size.…”

“…However, untreated hypothyroidism has been shown to increase the risk of heart failure 48,49 as well as coronary heart disease (CHD) and CHD-related death 10 . In this study, adherent patients were found to be more likely to have comorbid dyslipidemia, despite the fact that LT4 treatment usually corrects dyslipidemia 50 . However, in cases where LT4 therapy does not correct dyslipidemia, other factors, such as genetics, age or diet, may be involved 50 .…”

“…It was observed that pulse rate and respiratory rate had no significant difference with age groups while both systolic and diastolic blood pressures show significant variations (p value<0.05) with higher blood pressure in 36-45 years age group. 14 (Table 3) T3 has direct vasodilator action on vascular smooth muscle cells, so in hypothyroidism due to reduced T3 levels, vascular smooth muscle tone increases and systemic vascular resistance increases. 8 Endothelial dysfunction also affects systemic vascular resistance.…”
