Using an echo-Doppler method (Quantascope), the hemodynamic profile of the calcium channel antagonist elgodipine (64 micrograms/kg, i.v.) was investigated in 22 patients with angina pectoris at rest and during exercise. A placebo control was used. At rest, elgodipine significantly decreased systemic vascular resistance as well as systolic and diastolic blood pressure, while increasing cardiac output and stroke volume. During supine bicycle exercise the constant workload, elgodipine significantly increased cardiac output and stroke volume, and decrease the rate-pressure-product (double product); the exercise systolic blood pressure was decreased without change in the diastolic component. Elgodipine significantly reduced the incidence and severity (self-rated pain score) of exercise-induced anginal systems. Heart rate was not affected by elgodipine, either at rest or during exercise. In particular, no negative inotropy could be inferred from the echo-Doppler data. In the elgodipine plasma concentration profile (HPLC), three phases of elimination with half-life times of less than 1 hour, between 3 and 7 hours, and between 10 and 24 hours may be distinguished, indicating a "shallow" and a "deep" compartment. The hemodynamic data indicate an intermediate pharmacodynamic profile of elgodipine, lying between that of other dihydropyridines and that od compounds such as verapamil or diltiazem.

1 Nicardipine is a new peripherally selective slow calcium channel blocker. Like other dihydropyridine derivatives it appears to function as a systemic arteriolar vasodilator, and causes a significant reduction in left ventricular afterload. 2 Unlike the papaverine derivatives, and certain dihydropyridines, it does not demonstrate discernable cardiodepression in clinical situations. This appears due to selective vascular and myocardial sparing actions together with sympathetic activation and consequent counter-regulating corrective actions. 3 It has also been shown to improve coronary blood flow and to influence favourably myocardial metabolism. 4 Although there is currently limited information available, the combination of this drug with 3-adrenoceptor blocking agents appears to optimise its haemodynamic profile. It may also be of benefit in the treatment of patients with left ventricular failure. 5 Further studies are required to confirm these findings and to establish the place of nicardipine in the therapeutic management of patients with coronary artery disease.

1 The efficacy and safety of nicardipine, 30 mg three times a day, were compared with atenolol, 100 mg once daily in a randomised, double-blind, 11-week, crossover study of 46 patients with stable angina pectoris. 2 Both drugs significantly decreased the frequency of angina and improved total exercise time, time to angina, and time to 1 mm ST-segment depression. No statistically significant differences were observed between nicardipine and atenolol in clinical responses or treadmill performance. 3 At maximum workload, the double product decreased significantly in patients taking atenolol and did not change in those using nicardipine. 4 During treatment with atenolol, statistically significant decreases were observed in heart rate and QTc interval; these parameters increased minimally on nicardipine. 5 The incidence and severity of adverse experiences associated with each treatment were similar and did not interfere with the study. The two major adverse experiences, myocardial infarction and sudden death, occurred only in patients receiving atenolol. 6 Nicardipine therapy compared favourably with atenolol therapy in patients with angina pectoris.
