Effects of nicardipine on cardiac volume at rest and during exercise‐ induced angina.

Abstract: The action of nicardipine on cardiac volume, both at rest and during exercise‐induced angina, was evaluated in 12 patients with angiographically‐proven coronary artery disease. Nicardipine given to patients at rest reduced systemic vascular resistance and mean arterial pressure and increased heart rate and cardiac index. The left ventricular filling pressure, ejection fraction (EF), end‐diastolic and end‐systolic volumes were unchanged. During supine bicycle exercise, the reduction in systemic arterial blood p… Show more

“…This profile seems to be a hybrid between calcium channel antagonists, such as verapamil or dilgazem on the one hand, and the dihydropyridines, on the other hand. Elgodipine seems to be similar to verapamil [21] or diltiazem [21,22] for the parameters HR, SBP, and DBP, and to amlodipine [21,23], nisoldipine [20], nicardipine [24], and isradipine [25]) for SVR, CO, and SV, investigated with similar patients and design.…”

“…These observations were extended in a further dose-response study with the drug (Silke et al, 1985a). In addition to the description of the haemodynamics of nicardipine, the left ventricular ejection fraction and volume were measured using a non-imaging scintillation probe (Nuclear Stethoscope-Bios Inc).…”

“…Their efficacy appears to be achieved by relaxation of arterial wall smooth muscle leading to vasodilatation, hence reducing cardiac work and myocardial oxygen demand (Braunwald, 1982;Fleckenstein, 1977;Grossman & Barry, 1980;McCredie et al, 1985;Singh, 1982;Stone et al, 1980;Visser et al, 1985). Whilst some calcium antagonists have been shown to have a negative inotropic effect, this is by no means as great as that associated with f-adrenoceptor blocker treatment (Silke et al, 1985).…”
