Effects of Non-surgical Periodontal Therapy on Saliva and Gingival Crevicular Fluid Levels of Chemerin in Periodontitis Subjects With and Without Type 2 Diabetes Mellitus

Abstract: BackgroundEvidence had shown a bi-directional link between diabetes mellitus and periodontitis. Chemerin, an adipose tissue-specific adipokine plays a significant role in adipocyte initiation and differentiation that directly influences glucose metabolism, lipid metabolism, and inflammatory mediators. Non-surgical periodontal therapy (NSPT) for patients with periodontitis and diabetes mellitus improves the periodontal condition and regulates glycemic level.

“…The description of non-diabetes varied among the studies included in this review. More than half of the studies (19/34) defined non-diabetics subjects as systematically healthy individuals or those without a diagnosis of DM [36,37,[39][40][41][45][46][47]51,52,[58][59][60][63][64][65][66][67][68]. Furthermore, the glycemic test parameters and cut-off values varied from 6% to 6.5% for HbA1c [35,43,[48][49][50]53,[55][56][57]61,76], 100 to 126 mg/dl for FPG [35,38,42,44,48,50], and 140 to 200 mg/mL for RBS [43,56,62].…”

“…The definitions of periodontitis differed among the studies included in this review (Table 1). The American Academy of Periodontology (AAP) 1999 classification was used to diagnose periodontitis in 20 (58.8%) studies, whereas 4 studies [35,58,59,61] used the American Academy of Periodontology and European Federation of Periodontology (AAP/EFP) 2017 classification; meanwhile, 2 studies [37,38] used the Community Periodontal Index (CPI), and in 1 study [50], the disease was diagnosed based on the criteria from the Center for Disease Control and Prevention and the American Academy of Periodontology (CDC/AAP). Six sites per tooth were examined in more than half of the studies, whereas four sites per tooth were examined in two studies [47,66].…”

“…Systemic administration offers the potential benefit of effectively targeting pathogens that have spread extensively throughout the oral cavity, including those in non-dental oral areas, such as the tongue's upper surface and the tonsils' crevices. Still, it requires a lot of patients to follow through with their treatment, can introduce unwanted systemic side effects that can make it harder for patients to do so, and may help bacteria become more resistant [7][8][9]. On the other hand, local administration is not influenced by patient adherence.…”
