Effects of Nutritional Supplementation on Prealbumin Concentrations in Pediatric Burn Patients: A Randomized, Controlled Trial

Abstract: Abstract:Introduction: This prospective, randomized study was designed to measure the effect of nutritional supplementation on serum prealbumin (transthyretin) in pediatric burn patients in seven burn centers in Russia. The primary measure was change in serum prealbumin from baseline to Day 7.Methods: Fifty-four children age 1-10 years with a TBSA burn of at least 15% were enrolled into this study. Patients had baseline serum prealbumin levels < 10 mg/dL and were capable of exclusive oral feeding within 72 hou… Show more

“…A total of 64 patients were assessed in terms of entry criteria, and 7 patients were excluded as they did not meet the entry criteria. Of the 51 patients who were randomly assigned to the intervention group (26) and control (25), 17 participants were excluded from the study due to various reasons; 7 patients due to death, 5 due to intolerance, 3 due to cessation of feeding, and 2 due to changes in the feeding method during the intervention(Figure 1). So, nally 34 participants (17 people in the intervention group (7 men and 10 women) and 17 patients in the control group (9 men and 8 Women) completed the study for a week and were statistically analyzed.…”

“…However, our study is the rst study to compare albumin responses to two methods of continuous feeding and bolus feeding. In a study of burn patients, Williams et al [25] showed that nutritional adequacy can be measured by increasing prealbumin levels. In trauma and burn patients, prealbumin level is the most reliable indicator of nutritional adequacy [25].…”

“…[ 25 ] In a clinical trial (2011) on burned children in Russia, it was shown that appropriate nutrition was accompanied with an increase in the prealbumin level. [ 15 ] In another study on the ICU patients, a notable increase in the level of patients’ prealbumin level was observed after adequate and appropriate nutrition, compared with the subjects without adequate nutrition. [ 26 ]…”
