1 Sodium metabisulphite (MBS) can induce bronchoconstriction in patients with asthma. We investigated the effects of MBS aerosol on bronchial blood velocity (Vbr) and pulmonary resistance in intubated conscious sheep. 2 Bronchial blood velocity was measured by implanting a 20 MHz ultrasonic Doppler flow probe on the common bronchial branch of the bronchoesophageal artery. 3 Inhaled MBS induced a dose-dependent, transient increase in Vb. lasting for a few minutes without any changes in aortic and pulmonary artery pressures. There was some tachyphylaxis of the Vbr response to successive inhalations of MBS. 4 The cholinoceptor antagonist, ipratropium bromide and the H, and H2 histamine antagonists, chlorpheniramine and cimetidine, had no significant effect on MBS-induced increase on Vbr. The loop diuretic, frusemide, and the anti-inflammatory drug, nedocromil sodium, which both inhibit MBSinduced bronchoconstriction in patients with asthma, were, also without effect. 5 We conclude that MBS induces bronchial vasodilatation in conscious sheep, and that this effect is not dependent on the release of histamine or other mediators, or an activation of cholinergic pathways.

Objectives-To screen adult subjects with asthma for sensitivity to inhaled sulphur dioxide (SO 2 ) and identify subject characteristics associated with that sensitivity. Medication use, symptoms, and plasma antioxidant nutrients between SO 2 responders and non-responders were compared. Methods-Adult subjects (ages 18-39 years) with asthma were exposed to 0.5 ppm SO 2 for 10 minutes during moderate exercise. Pulmonary function tests and symptom ratings were assessed before and after exposure (n=47). A subject was classified as sensitive to SO 2 if forced expiratory volume in 1 second (FEV 1 ) showed a drop >8% over baseline. Blood samples were obtained from subjects (n=38) before the SO 2 challenge; plasma ascorbate, -tocopherol, retinol, carotenoids, and lipids were measured. Results-Of the 47 subjects screened, 53% had a drop in FEV 1 >8% (ranging from −8% to −44%). Among those 25 subjects, the mean drop in FEV 1 was −17.2%. Baseline pulmonary function indices (FEV 1 % of predicted and FEV 1 /FVC% (forced vital capacity)) did not predict sensitivity to SO 2 . Although use of medication was inversely related to changes in pulmonary function after SO 2 (p<0.05), both SO 2 responders and non-responders were represented in each medication category. Total symptom scores after exposure were significantly correlated with changes in FEV 1 (p<0.05), FVC (p<0.05), and peak expiratory flow (PEF) (p<0.01) but not forced expiratory flow between 25% and 75% vital capacity (FEF

Resumo: O organismo humano está exposto às mais diversas agressões do ambiente; acidentes, agressões físicas, químicas e microbiológicas constituem um permanente risco para o organismo. O aparelho respiratório está particularmente exposto às agressões do ambiente e é freqüentemente sede de alterações de maior ou menor intensidade e gravidade. Revêem-se neste trabalho os efeitos da poluição atmosférica sobre o aparelho respiratório. Definem-se poluição atmosférica, poluição do exterior e dos interiores e poluição primária e secundária. Revêem-se sumariamente os mecanismos de defesa do aparelho respiratório e descrevem-se as ações deletérias dos diferentes poluentes atmosféricos para as vias aéreas: SO 2 , NO 2 , CO, O 3 , aerossóis e partículas. Destaca-se a ação do fumo do tabaco atendendo à sua importância para a saúde. Termina-se tecendo alguns comentários sobre a intervenção que a pneumologia e os pneumologistas devem ter nesta área, chamando a atenção para a comunidade e para as autoridades, para os riscos da poluição para a saúde em geral e, em particular, para o aparelho respiratório.
