Efficacy and safety of exchange transfusion as an adjunct therapy for severe Plasmodium falciparum malaria in nonimmune travelers: a 10‐year single‐center experience with a standardized treatment protocol

Abstract: No case fatalities were observed in the ET group. The complications that were observed with ET were more likely related either to the multiorgan dysfunction associated with severe malaria or to side effects of parenteral quinine rather than to the ET procedure. ET may be safely executed in a setting with intensive care facilities and availability of safe blood products and should be considered as a beneficial adjunct treatment to parenteral antimalarial therapy.

“…In striking contrast, in non-endemic industrialized countries malaria is seen as an occasionally imported disease in non-immune travellers, but it still represents a potentially fatal disease [2,3]. Without prompt and proper treatment malaria may rapidly progress to complications and even death.…”

“…ET with healthy donor erythrocytes may improve microvascular diseases and oxygen-carrying capacity. 8 Published data about the effectiveness of ET is controversial. Several case reports have described the successful use of ET as an adjunctive therapy for severe malaria.…”

“…Case fatalities were not observed in the ET group. 8 Adverse events of ET were rarely reported, but included fluid overload, transfusion reactions, metabolic disturbances like hypocalcemia, transmissible infection, bleeding, red blood cell alloantibody sensitization, acute respiratory distress syndrome, ventricular fibrillation, and hypotension. 13 Any adverse events did not develop in our patient.…”

“…İlaç tedavisinin yetersiz kaldığı vakalarda terapötik eritrositoferez tedavisinin başarılı olduğu bildirilmiştir (14,15). Bu olgulardan birinde de tedaviye yeterli yanıt alınamayınca Exchange Transfussion(ET) uygulanmış ve başarılı sonuç alınmıştır.…”
