Purpose We conduct this study to compare the efficacy and toxicity of intensity-modulated radiotherapy (IMRT) concurrent weekly nedaplatin (NDP) versus IMRT alone in the stage III/IV non-surgical elderly patients with nonsmall-cell lung cancer (NSCLC). Methods 117 patients were enrolled into our study. The patients were assigned into two different groups: radiotherapy (RT) group and chemoradiotherapy (CRT) group. Patients in RT group were treated with IMRT at a single daily dose of 2 Gy for 5 days per week, totally 52-66 Gy. The CRT group, IMRT concurrent weekly NDP at a dose of 25 mg/m 2 . Results In CRT group, the median survival was 11.0 months (95% confidence interval [CI], 8.894-13.106 months) and in RT group, it was 7.0 months (95% CI 5.771-8.229 months). The 1-year, 2-year, 3-year, survival rates in the combined treatment arm were higher than the radiation therapy arm (46.8 vs 25.9%, 25.1 vs 11.8%, 14.7 vs 8.0%; p \ 0.001). The Cox's multiple regression analysis showed that CRT had significantly better overall survival than RT (HR 0.523; 95.0% CI 0.338-0.807; p = 0.003). The objective response rate provided that 73.3% treated with CRT compared with 51.1% (p = 0.018) received RT alone. Of the hematologic toxicities, leukocytes (35.0 vs 0%; p \ 0.001), neutrophils (33.3 vs 0%; p \ 0.001) were significantly more common in the CRT group than the RT group. Conclusions We first discovered that NDP concurrent IMRT for treating stage III/IV non-surgical elderly patients with NSCLC was good curative effect of better objective response rate and well-tolerated. However, within the low number of patients, only stage IV gained a survival benefit.

The present study aimed to investigate the effects of S-1 combined with palliative care (PC) on the immune function and quality of life (QOL) of patients with advanced stomach cancer (ASC). In this prospective study, 168 patients with ASC admitted to our hospital from September 2016 to March 2018 were enrolled as research objects. Seventy-seven cases were treated with S-1 alone (single drug group, SDG), while another 91 cases were treated with S-1 combined with PC (combined drug group, CDG). The effects of the two therapeutic methods on the efficacy [overall response rate (ORR)], 1-year overall survival rate (OSR), safety, negative emotions, nutritional indices, QOL, and immune function indices of patients were analyzed. After treatment, ORR, OSR, levels of nutritional indices [albumin (ALB), prealbumin (PA), and transferrin (TF)], and QOL improvement rate in the CDG were significantly higher than those in the SDG (P<0.05). After treatment, compared with those in the SDG, patients in the CDG had a lower Self-Rating Anxiety Scale (SAS) score, Self-Rating Depression Scale (SDS) score, and number of adverse reactions (P<0.05), and significantly improved immune function indices (CD4 + , CD8 + , and CD4 + /CD8 + ) (P<0.05). S-1 combined with PC treatment was superior to S-1 treatment alone in patients with ASC. The patients treated with the combination exhibited improved efficacy (a higher ORR), higher QOL, and improved immune function, and thus this treatment can be clinically popularized.

The combined use of tegafur (TAR), gimeracil (GAR), and oteracil (OAR) is often employed for the therapy of malignant tumors. A high-performance liquid chromatography (HPLC) approach was developed to consistently and precisely measure GAR, OAR, and TAR in a combined pharmaceutical formula that uses a shorter operation time. The separation followed by assessment of GAR, OAR and TAR is done with C18 (Waters, USA) column (250; 4.6 mm; temperature 25˚C) and the mobile phase ratio used included 60% vol (0.1 M NaHSO4): 40% vol (methanol). Upon applying stress conditions, International Council for Harmonisation (ICH) recommended to GAR, OAR and TAR. The HPLC findings pointed out the nonexistence of any interference between the drugs under test and the degradation compounds. The stability indicating quality was established by the peak purity results for GAR, OAR, and TAR, which disclose that the test peaks (GAR, OAR, and TAR) were homogenous in all stress settings examined. From stability studies, we found that GAR showed significant degradation when rendered to dry heat, while TAR and OAR exhibited significant degradation after being exposed to acidic conditions. The suggested approach could potentially be implemented to assess the stability of GAR, OAR, and TAR under stress environments as well as to perform quality control checks on the three mentioned drugs in capsule doses.
