Background Allergic rhinitis (AR) is an IgE-mediated disease that adversely affects quality of life. Many studies report that moxibustion is an effective treatment for perennial allergic rhinitis (PAR). However, it is difficult to perform moxibustion on the face because of possible burning of the skin and the noxious effects of smoke. Electric heating moxibustion does not have these limitations. The purpose of this clinical trial is to assess the possibility of treating PAR with electric heating moxibustion and to assess the feasibility of conducting a clinical test on a larger scale. Methods This is a randomized, open-label, assessor-blind, parallel-design pilot clinical study. We will recruit 40 eligible participants and randomly allocate them into an electric heating moxibustion group or an acupuncture group at a 1:1 ratio. Patients in both groups will receive eight treatments over 4 weeks, and the final follow-up will be 4 weeks after the last treatment. Eleven acupuncture points will be used for patients in both groups (EX-HN3 and bilateral EX-HN-8, LI20, LI4, GB20, and ST36). The primary outcome measure is change in the Total Nasal Symptom Score, and the secondary outcome measures are changes in the Rhinoconjunctivitis Quality of Life Questionnaire, nasal endoscopy index for pattern identification, pattern identification questionnaire for AR, total IgE, eosinophil count, and adverse effects. Discussion This clinical trial will examine the effect of electric heating moxibustion on PAR. Trial registration ClinicalTrials.gov, NCT03342105 . Registered on 14 November 2017. Introduction: Allergic rhinitis has a severe impact on patients' life quality, and the incidence rate keeps increasing. Moxibustion is widely used for treating allergic rhinitis, and quantity is the basis of moxibustion efficacy. The purpose of this study is to evaluate the relationship between the different quantities of moxibustion and the efficacy of moxibustion in the treatment of allergic rhinitis. This study may be conductive to the standardization of moxibustion and furnish the mechanism of dose-effect relationship of moxibustion with data and new ideas. Methods and analysis: This randomized, placebo-controlled clinical trial will include 33 patients with allergic rhinitis who will be randomly assigned into three groups in a 1:1:1 ratio：high-dose moxibustion group, low-dose moxibustion group and sham moxibustion control group. All groups will be treated once every other day, 20 days for one treatment course. And the patients will receive treatment for 2 courses with an interval of 2 days between courses. We will conduct a follow-up 30 days later after completion of treatments. The primary outcome measure is Total Nasal Symptom Score, carried out at baseline, 3, 6 and 10 weeks. Secondary outcome measure is Rhino Conjunctivitis Quality of Life Questionnaire, carried out at baseline, 6 and 10 weeks. Ethics and dissemination: This trail has been approved by the IRB of Shanghai TCM-Integrated Hospital, Shanghai University of TCM. The results of the trial will be disseminated in peer-reviewed journals. Trial registration number: ChiCTR2100050373; Pre-results.

Allergic rhinitis (AR) is an immunoglobulin E–mediated illness that causes inflammation of the nasal mucosa. In recent years, the global incidence and prevalence of AR has been increased. AR has been treated with antihistamines, intranasal corticosteroids, decongestants, intranasal anticholinergics, intranasal cromolyn, leukotriene receptor antagonists, and immunotherapy. However, because many of the present medicines have documented negative side effects, there is a need to look for more effective and safer alternatives. Herbal therapy has long been used to treat the symptoms of AR, but its usefulness and safety have not been validated scientifically. In this review, recent research on the use of herbal medicine as a treatment for allergic rhinitis is collated and critically analyzed. From 2016 to 2021, the data was gathered mostly from English language publications published in journals or research from Pubmed databases. "Herbal medication" and "Allergic Rhinitis" were the terms or keywords used to discover relevant studies. This literature study may be useful in determining the potential utility of herbal medicine as a treatment for allergic rhinitis, determining the underlying mechanisms of action, and identifying natural product candidates for the development of innovative anti-allergic rhinitis drugs in the future.
