Efficacy and Safety of Tolvaptan in Patients More Than 90 Years Old With Acute Heart Failure

Abstract: Background: The oral vasopressin-2 receptor antagonist, tolvaptan, causes aquaresis and improves symptoms in patients with congestive heart failure. However, few studies have explored the effect of tolvaptan in acute heart failure (AHF) patients ≥90 years old. Methods: This study enrolled 106 AHF patients treated with tolvaptan added to standard therapy. The subjects were divided into 2 groups: ≥90-year-old patients (over-90 group, mean 92 ± 2 years, n = 45) and <90-year-old patients (under-90 group, mean 7… Show more

“…HF was categorized as ‘HF preserved ejection fraction’ (HFpEF) (left ventricular ejection fraction [LVEF] ≥ 50%), ‘HF mildly reduced EF’ (HFmrEF) (LVEF 40–49%), and ‘HF reduced EF’ (HFrEF) (LVEF <40%) 1 . The doses of 60 mg azosemide and 10‐mg torasemide were equivalent to 40 mg furosemide 7 . The D2F time was defined as the time from arrival of a patient at a participating hospital to the first administration of furosemide 4 …”

“…In Japan, this drug has been used, together with loop diuretics, in volume overload HF patients since 2010 5 . Over the past few years, several studies have been conducted to investigate the favourable effects of tolvaptan on clinical outcomes in HF patients under various situations 6,7 . Early initiation of tolvaptan, within a couple of days, provided immediate decongestion and better clinical outcomes, such as prevention of worsening renal function (WRF) and shortening of the length of hospital stay 8–10 …”

“…In recent years, TLV has been used as one of the therapeutic agents for HF patients with volume overload along with loop diuretics in Japan. The efficacy and safety of TLV in elderly patients with HF have been reported in the Surveillance In Heart faiLurE (SMILE) study and other previous studies [10][11][12][13]. However, few studies have investigated the outcomes of elderly patients treated with TLV during the hyperacute phase of HF.…”

“…TLV combined with loop diuretics has been shown to be effective in the treatment of diuretic-resistant AHF patients ( 6 , 15 , 16 ). However, there are few reports on whether TLV is equally effective in elderly patients with AHF and oliguria ( 10 , 17 ). For this reason, in this study, we examined the efficacy and safety of TLV in elderly patients with AHF and oliguria.…”
