Background Multiple pharmacological interventions and modalities are available for managing chronic idiopathic constipation (CIC), with variable efficacy. Vibrating capsule (VC) is a device that has shown variable results in alleviating constipation by tactile stimulation of the colonic wall and inducing peristalsis. This meta-analysis is to investigate the efficacy and safety of this modality. Methods Comprehensive literature search was performed through June 14th, 2023, on databases including Embase, PubMed/MEDLINE, Cochrane Central, Web of Science, Global Index Medicus, and Google Scholar. Core concepts of VC, constipation, and bowel movement were searched. The DerSimonian-Laird method and random effects model were utilized. We calculated odds ratio (OR) and mean difference (MD) for proportional and continuous variables, respectively, with 95% confidence interval (CI) and a P value of <0.05 considered statistically significant. Results The search strategy yielded 117 articles. Four studies with 705 total patients were finalized comparing VC to placebo/sham treatment. The pooled complete spontaneous bowel movement (CSBM), defined as bowel movement without use of laxatives within the last 48 hours with sense of complete evacuation did not achieve statistical improvement with VC (MD =0.153; 95% CI: −0.218 to 0.523; P=0.422). However, spontaneous bowel movement (SBM), defined as bowel movement without use of laxatives within the last 48 hours, showed statistical improvement with VC (MD =0.159; 95% CI: 0.095 to 0.223; P<0.001). VC didn’t show an increase in pooled adverse events (OR =1.431; 95% CI: 0.702 to 2.916; P=0.324). Conclusions The systematic review and meta-analysis suggest that VC is safe and efficacious in some outcomes, however, larger randomized controlled trials (RCTs) and real-world data are needed to establish this.

SummaryBackgroundAdvances in microelectronics have greatly expanded the capabilities and clinical potential of ingestible electronic devices.AimTo provide an overview of the structure and potential impact of ingestible devices in development that are relevant to the gastrointestinal tract.MethodsWe performed a detailed literature search to inform this narrative review.ResultsTechnical success of ingestible electronic devices relies on the ability to miniaturise the microelectronic circuits, sensors and components for interventional functions while being sufficiently powered to fulfil the intended function. These devices offer the advantages of being convenient and minimally invasive, with real‐time assessment often possible and with minimal interference to normal physiology. Safety has not been a limitation, but defining and controlling device location in the gastrointestinal tract remains challenging. The success of capsule endoscopy has buoyed enthusiasm for the concepts, but few ingestible devices have reached clinical practice to date, partly due to the novelty of the information they provide and also due to the challenges of adding this novel technology to established clinical paradigms. Nonetheless, with ongoing technological advancement and as understanding of their potential impact emerges, acceptance of such technology will grow. These devices have the capacity to provide unique insight into gastrointestinal physiology and pathophysiology. Interventional functions, such as sampling of tissue or luminal contents and delivery of therapies, may further enhance their ability to sharpen gastroenterological diagnoses, monitoring and treatment.ConclusionsThe development of miniaturised ingestible microelectronic‐based devices offers exciting prospects for enhancing gastroenterological research and the delivery of personalised, point‐of‐care medicine.

Advances and updates in medical applications utilizing microwave techniques and technologies are reviewed in this paper. The article aims to provide an overview of enablers for microwave medical applications and their recent progress. The emphasis focuses on the applications of microwaves, in the following order, for 1) signal and data communication for implants and wearables through the human body, 2) electromagnetic energy transfer through tissues, 3) noninvasive, remote or in situ physical and biochemical sensing, and 4) therapeutic purposes by changing tissue properties with controlled thermal effects. For signal and data communication and wireless power transfer, implant and wearable applications are discussed in the categories of pacemakers, endoscopic capsules, brain interfaces, intraocular, cardiac and intracranial pressure sensors, neurostimulators, endoluminal implants, artificial retina, smart lenses, and cochlear implants. For noninvasive sensing, remote vital sign radar, biological cell probing, magnetic resonance and microwave imaging, biochemical, blood glucose, hydration and biomarker sensing applications are introduced. For therapeutic uses, the developments of microwave ablation, balloon angioplasty, and hyperthermia applications are reviewed. The scopes of this article mainly concentrate on the research and development efforts in the past 20 years. Recent review articles on specific topics are cited with accomplishment highlights and trends deliberated. At the end of this article, a brief history of the IEEE Microwave Theory and Techniques Society (MTT-S) Biological Effects and Medical Applications committee and the contributions by its members to the promotion and advancement of microwave technologies in medical fields are chronicled.
