Efficacy and safety of VNS therapy or continued medication management for treatment of adults with drug-resistant epilepsy: systematic review and meta-analysis

Abstract: Vagus nerve stimulation (VNS) Therapy® is an adjunctive neurostimulation treatment for people with drug-resistant epilepsy (DRE) who are unwilling to undergo resective surgery, have had unsuccessful surgery or are unsuitable for surgery. A systematic review and meta-analysis were conducted to determine the treatment effects of VNS Therapy as an adjunct to anti-seizure medications (ASMs) for the management of adults with DRE. A literature search was performed in August 2020 of the Medline®, Medline® Epub Ahead … Show more

“…While VNS has helped tens of thousands of patients with chronic disease (primarily refractory epilepsy), a persistent challenge is the therapy’s lack of precision. Response rates range between 50-70% (Englot et al, 2015; Toffa et al, 2020; Batson et al, 2022) and side effects are common, including cough, voice alteration, laryngeal spasms, and local pain Ben-Menachem, 2001. Additionally, it can take up to 12 months of adjusting stimulation parameters before peak effectiveness is reached (LivaNova, 2020).…”

“…Acute and chronic deep brain stimulation (DBS), as well as vagus nerve stimulation (VNS), are epilepsy therapies (236,(246)(247)(248). Combining neurostimulation and ASM may be more effective in controlling seizures than either alone (249).…”
