Efficacy and safety profile of xanthines in COPD: a network meta-analysis

Abstract: Theophylline can still have a role in the management of stable chronic obstructive pulmonary disease (COPD), but its use remains controversial, mainly due to its narrow therapeutic window. Doxofylline, another xanthine, is an effective bronchodilator and displays a better safety profile than theophylline. Therefore, we performed a quantitative synthesis to compare the efficacy and safety profile of different xanthines in COPD.The primary end-point of this meta-analysis was the impact of xanthines on lung funct… Show more

“…Specifically, the numerator represents the count of total events and the denominator represents the given time duration multiplied by the number of patients [22]. Such a method has been supported by the Cochrane Collaboration and successfully used in recent meta-analyses [20, 21, 23, 24].…”

“…Pharmacological trials where the statistical significance matches clinical provide the greatest probability that the investigated treatment will be effective in clinical practice . Interestingly, the concept of MCID has been only recently investigated in meta‐analyses in human respiratory medicine . The MID based on change in R L can be used in pharmacological trials to identify responder asthmatic horses to a bronchodilator agent and this approach will permit deep analyses of results beyond the usual comparison of groups .…”

“…Notwithstanding the above-mentioned limitations, this pairwise meta-analysis suggests that in patients with COPD doxofylline acts as a bronchodilator drug having a wider therapeutic window than theophylline. These characteristics, along with its well-described anti-inflammatory [48] and steroid sparing effects [49] suggest that when a xanthine is indicated [50], doxofylline should be considered as an alternative to theophylline in the treatment of patients with COPD.…”
