Efficacy of 6‐mm diameter fully covered self‐expandable metallic stents in preoperative biliary drainage for pancreatic ductal adenocarcinoma

Abstract: Objectives Plastic stents (PS) used for preoperative biliary drainage (PBD) of pancreatic ductal adenocarcinomas (PDAC) tend to be associated with a high incidence of recurrent biliary obstruction (RBO). Although 10‐mm diameter fully covered self‐expanding metallic stents (FCSEMS) have come into use, vigilance is still required to prevent complications, such as cholecystitis and surgical site infection. Kataoka et al [28] compared the outcomes of a 6-mm FCSEMS and 7-8.5-French PS retrospectively in patients with preoperative pancreatic cancer; they found that TRBO was longer in the 6-mm FCSEMS group (P=0.02) than in the 7-8.5-French PS group, and stent-related complications were not significantly different between the groups (P=0.47). In summary, a 6-mm FCSEMS could be a well-balanced stent that reduces the risk of stent-related adverse events as much as a PS, while ensuring TRBO comparable to the standard 10-mm FCSEMS.…”
