Low-dose isotretinoin (0.5 mg/kg/day) is a mode of therapy for mild to moderate grades of acne. We analyzed the various trials of this mode of therapy with or without combination with topical agents. We also statistically analyzed the results, efficacy and relapse rates of standard therapy in comparison with the low-dose therapy. Our analysis of the data revealed that the efficacy and relapse rates of low-dose isotretinoin in mild to moderate grades of acne is comparable with the standard regimen (1 mg/kg/day), which is given in the severe grade of acne vulgaris. Thus, the grade of acne vulgaris should dictate the dose of administration of isotretinoin and the standard dose of 1 mg/kg/day is an unnecessary overtreatment for mild to moderate grades of acne.

Randomized comparative study of combination of low dose oral isotretinoin with pulsed azithromycin and low dose oral isotretinoin alone in the management of moderate to severe acne

Background: Oral isotretinoin and azithromycin are two important drugs used in management of acne. Their mode of action is different and thus theoretically the combination of these two should have a synergistic effect.Methods: In this randomized comparative trial 100 patients were randomized into two groups. One received oral isotretinoin (0.3 mg/kg/day) alone and the other group received pulsed azithromycin (500 mg/day for three days in a week for 8 weeks) in addition to oral isotretinoin. Improvement in acne was noted as percentage improvement in mean acne score.Results: No statistically significant difference was noted in the improvement in mean acne score of both the groups.Conclusions: Addition of pulsed azithromycin to low dose oral isotretinoin does not offer a better treatment outcome in patients of moderate to severe acne.

КОРсУнсКая 1 ФГБУн «центр теоретических проблем физико-химической фармакологии Российской академии наук», Москва, Россия, 119991 В работе приведен обзор и собственные наблюдения авторов по применению изотретиноина в различных дозах для терапии тяжелых и торпидных к терапии форм акне. Подчеркнута особая роль информированности пациентов и врачей смежных специальностей о ретиноидах для адекватного подхода к терапии. Описаны различные алгоритмы назначения изотретиноина, приведены рекомендации отечественных и зарубежных специалистов. Полученные собственные результаты свидетельствуют о возможности длительного применения изотретиноина по ступенчатой схеме снижения доз, что позволяет избежать негативных эффектов от проводимой терапии и увеличить срок ремиссии. The paper provides the review and our own observations on the use of different doses of isotretinoin in treatment of severe and torpid forms of acne. We emphasized the special role of information awareness of patients and doctors, who work in the allied health specialties, about retinoids to provide an adequate approach to therapy. Various algorithms of isotretinoin administration were discussed and recommendations of Russian and international experts are provided. our own results show that long-term administration of isotretinoin is possible with stepwise dose reduction regimen, which prevents from negative effects of the therapy and increased duration of remission.

Abstract: Six months of treatment with fixed-dose, alternate-day isotretinoin (20 mg) plus topical 1%clindamycin gel was found to be effective in the treatment of moderate acne in adult patients, with a low incidence of side-effects.
