Efficacy of intercostal nerve block with 0.25% bupivacaine in percutaneous nephrolithotomy: A prospective randomized clinical trial

Abstract: Introduction and Aim:Several techniques have been used to lower the morbidity of percutaneous nephrostomy (PCN) tube after percutaneous nephrolithotomy (PCNL). The outcomes of intercostal nerve block (ICB) versus peritubal block (PTB) with 0.25% bupivacaine to alleviate post-PCNL pain were compared.Materials and Methods:After obtaining an informed written consent and local institutional ethics clearance, 64 patients undergoing PCNL were computer randomized to receive either an intercostal block/ICB (Group I) o… Show more

“…Bupivacaine and ropivacaine were used for the ICNBs. Two studies ( 7 , 18 ) compared ICNB with PT infiltration, three ( 13 , 20 , 21 ) compared with control while one study ( 19 ) was a three-arm trial including ICNB, PT infiltration, and control. All studies administered ICNB after the procedure, except for Ozkan et al ( 20 ).…”

“…Clinicians have also reported that peritubal (PT) infiltration of the nephrostomy tract with an anesthetic can reduce post-operative pain ( 6 ). Amongst nerve blocks, paravertebral, and intercostal nerve block (ICNB) are commonly used to provide post-operative pain relief after PCNL ( 7 , 8 ).…”

“…The application of regional anaesthesia is known to possess the highest efficacy in managing pain following surgery of this sort [ 2 ]. There are a number of methods available which may potentially reduce postoperative pain associated with nephrostomy tube placement in PCN, namely intercostal nervous blockade, epidural analgesia, peritubal infiltration of local anaesthetic and paravertebral blockade [ 3 5 ]. Paravertabral blocks (PVB) are in use to adequately manage pain arising from a variety of operations on the thorax, abdomen or pelvis.…”
