Background: Cardiovascular disease is the leading cause of death among people with kidney failure on hemodialysis, for whom improving cardiovascular health is a research priority. Intradialytic myocardial stunning is common and associated with adverse cardiovascular events. Intradialytic exercise may mitigate intradialytic myocardial stunning and improve cardiovascular structure and function. This systematic review investigated the effect of intradialytic exercise on cardiovascular outcomes in adults undergoing maintenance hemodialysis (PROSPERO #CRD42018103118). Methods: Following PRISMA guidelines, we systematically searched MEDLINE, EMBASE, Cochrane CENTRAL, SportDiscus, and PEDro databases from 1960 until June 2022, for randomized and non-randomized studies investigating the effect of intradialytic exercise programs on objective cardiovascular outcomes, pre-specified as primary or secondary outcomes. The primary outcome was arterial resistance. Results: Of 10837 references identified, 32 met eligibility criteria. These studies investigated the effect of intradialytic exercise on arterial resistance (8 studies), blood pressure (20 studies), myocardial structure and function (7 studies), endothelial function (2 studies), sympathetic overactivity (9 studies), biomarkers of cardiac injury (3 studies), cardiovascular hospitalization and mortality (2 studies). Most studies used aerobic exercise as the intervention and usual care (no exercise) controls. Meta-analysis of intradialytic exercise versus usual care resulted in a statistically significant reduction in arterial resistance measured by pulse wave velocity (PWV) with mean difference -1.63 m/sec (95% CI -2.51, -0.75). Meta-analyses for diastolic blood pressure, left ventricular ejection fraction (LVEF), and low-frequency/high-frequency (LF/HF ratio) measure of heart rate variability also showed statistically significant improvements with exercise. There was no significant difference in change in systolic blood pressure, augmentation index, and left ventricular mass index between groups. Conclusion: Intradialytic exercise programming resulted in a clinically meaningful improvement to PWV, a component of arterial resistance. Sseveral physiologic measures of cardiovascular health, including diastolic blood pressure, LVEF and heart rate variability measured by the LF/HF ratio were also observed. The effects of intradialytic exercise on major adverse cardiovascular events remains uncertain.

In India, end-stage renal disease (ESRD) complications have attained pervasive dimensions, and it is estimated that rates of illness will continue to increase in the future. When both kidneys are failing, it is like being sentenced to death and lifetime hemodialysis (HD) is the only possible solution. Hardly 10% of sufferers from ESRD throughout India are obtaining HD because of the difficulties encountered. The ESRD catastrophe in the young generation of working professionals is a huge socio-economical concern. End-stage renal failure is a result of various non-contagious ailments. As per the worldwide incidence of illness, chronic renal impairment was the ninth greatest standard trigger of the disease in India. Annually in India, encounter about 2.2 lakhs new patients with kidney disorders. The main impediments in obtaining proper therapy inter alia are enormous costs in corporate hospitals, facilities for the treatment in metro cities, a confined number of Nephrologists, etc. Approximately 4950 hemodialysis hubs are located under corporate stakeholders in India. Every single hemodialysis costs approx. INR 2000, which when calculated costs about INR 3-4 LPA. HD is effective medical care for ESRD. The outlay is at all the time important problem for HD sufferers. Moreover, the majority of the hemodialysis patients require an attendant from family amounting to a further increase in the cost. It has been considered that in respect of both, determining these life-significant life-saving techniques as well as for diminishing destitution on the attention of ongoing costs for sufferers, a healthcare scheme is imperative for hemodialysis patients. Renal replacement therapy (RRT) treatment within India is largely a privatize medical assistance enterprise making this a costly therapy option because of huge out-of-pocket outlays. At this stage, the vast majority of HD sufferers in India die without obtaining a suitable prognosis. There are a handful of healthcare schemes provided by the Central Government, State Governments, NGOs, and even hospitals. However, there is a dire need to bring awareness among the public and the benefits that each scheme provides.

Background: Patients with chronic kidney disease (CKD) have less exercise capacity, and they are at higher risk of experiencing cognitive impairment, and lower health-related quality of life (QoL) than healthy individuals. Thus, the current study aims to assess the effect of exercise on patients’ cognition, QoL, and biochemical parameters. Methodology: This is an observational study to evaluate the effect of the exercise, which enrolled 90 patients with CKD (stage 3, 4, or 5) admitted to the Intensive Care Unit (ICU). Patients were made to do a 20-25 min exercise (breathing and limb exercises) and its effect on biochemical parameters (serum creatinine, sodium, potassium, and hemoglobin) was recorded from patient reports from day 1(T1; baseline) till day 7(T7). Further, the effect of exercise on cognition and QoL was assessed at T1 and T7 using the Mini-Mental State Exam (MMSE) and World Health Organization Quality of Life Instrument (WHOQOL-BREF), respectively. Results: Significant improvement in the level of serum creatinine, sodium, potassium, and hemoglobin were found in patients with CKD after 7 days of lower and upper limb and breathing exercises. Moreover, significant improvement in all four domains of WHOQOL-BREFi.e., physical health, psychological health, social relationship, and the environment was observed. Conclusion: This study showed that after 7 days of exercise, there was a significant improvement in cognition, biochemical parameters, and QoL in CKD patients.
