Efficacy of platelet‐poor plasma gel in combination with fractional CO2 laser in striae distensae: A clinical, histological, and immunohistochemical study

Abstract: Striae distensae (SD) are cosmetically unacceptable form of atrophic cutaneous scaring that may be associated with significant emotional distress. The pathophysiology of SD is not well established yet. However, they are supposed to be associated with impairment of fibroblasts function and aberration of connective tissue organization. Although many therapies have been tried for SD, results are often modest and there is no definite gold standard treatment modality. 1,2

“…22 Gad et al carried out a research to evaluate the advantage of using a combination of platelet-poor plasma gel and fractional CO 2 laser instead of using fractional CO 2 laser alone and observed significant clinical improvements while utilizing the former method, and side effects such as PIH were more prevalent in the latter. 23 According to our findings, even though both treatments were successful in achieving significant improvements in each session in comparison to their previous session based on VAS criteria, microneedling was seen to cause a more notable improvement in the introductory sessions, while fractional CO 2 laser seemed to bring a more significant improvement in the subsequent sessions. Therefore, it seems that a combination of both treatment modalities would be beneficial; this is in line with the findings of another study that aimed to compare the efficacy of treating SD with a combination of fractional CO 2 laser and microneedling versus sole treatment with the fractional CO 2 laser or microneedling.…”

“…Abdallah et al observed that using the fractional CO 2 laser combined with lyophilized growth factor intralesional injection instead of using the fractional CO 2 laser or lyophilized growth factor intralesional injection alone was noticeably more successful in treating SD 22 . “…Further, posttreatment histopathological analysis done by Nashwa et al in acne scars and Gad et al [ 16 ] in striae distensae showed a significant improvement in collagen bundles thickness, condensation, and orientation. PPP gel at the site of the injection induces a slight inflammatory response without producing tissue necrosis and granuloma hypersensitivity reaction, suggesting tolerability and biocompatibility of the plasma gel.…”

“…3 Fractional treatments, such as fractional radiofrequency (FRF) and ablative fractional laser (AFL), performed better in remodeling the dermis. 4,5 Recently, nonablative fractional laser (NAFL) and AFL (2940-nm Er:YAG laser, ablative 10 600 nm carbon dioxide fractional laser) have been shown to be effective in improving SD. 5,6 In 2018, our research team found a significant clinical benefit in utilizing a combination of 2940-nm Er:YAG AFL with LED-RL and rb-bFGF for treating SA.…”
