Efficacy of Serratus Anterior Plane Block Using Bupivacaine/ Magnesium Sulfate Versus Bupivacaine/ Nalbuphine for Mastectomy: A Randomized, Double-Blinded Comparative Study

Abstract: Background: Mastectomy is a common surgical procedure associated with intra and postoperative pain if untreated adequately will lead to chronic pain. Objectives: This study aimed to evaluate the efficacy of serratus anterior plane block using bupivacaine/magnesium sulfate versus bupivacaine/ nalbuphine for mastectomy. Methods: 40 ASA I and II female patients age 25 - 60 years underwent breast cancer surgery received the block before induction of general anesthesia were divided into two groups GBM (n = 20): (bu… Show more

“…Moreover, the remaining 25 full-text publications were carefully evaluated to check their eligibility. Furthermore, 12 trials were eliminated owing to the following reasons: they were case reports ( n = 1) [ 16 ]; SAPB was not the only intervention ( n = 5) [ 17 21 ]; they were conference abstracts ( n = 2) [ 10 , 22 ]; and SAPB was compared with other types of nerve blocks ( n = 4) [ 11 , 23 – 25 ]. Eventually, we included 13 eligible and authentic studies [ 12 , 26 37 ] in this meta-analysis.…”

“…Some phenanthrenes have mixed receptor actions, and great care should be taken in prescribing these drugs with pure agonists because of the unpredictability of analgesic effects and precipitation of explosive abstinence syndrome. Nalbuphine is a strong K-receptor agonist and a Mu-receptor antagonist, which causes respiratory depression at higher doses that is not reversed with naloxone (9,10). Buprenorphine is a long-acting phenanthrene derivative that is a partial Mu-receptor agonist and a K-receptor antagonist, which is FDA-approved for the management of opioid dependence (11).…”

“…Epidural and paravertebral blocks for peri-operative pain management for thoracic and abdominal surgeries are commonly used regional anesthetic techniques (7)(8)(9). To provide a longer duration of analgesia, a number of adjuvants, such as opi-oids, non-steroidal anti-inflammatory drugs, and alpha 2agonists have been used in conjunction with local anesthetics for peripheral and neuraxial blocks, including paravertebral blockade (10)(11)(12)(13)(14).…”
