Efficacy of Temporomandibular Joint Arthrocentesis on Mouth Opening and Pain in the Treatment of Internal Derangement of TMJ—A Clinical Study

Abstract: Purpose This study was designed to investigate the efficacy of arthrocentesis on mouth opening and pain in the treatment of temporomandibular joint (TMJ) internal derangement patients. Materials and Methods Fifteen males and 25 females aged between 18 and 37 years comprised the study material in the department of oral and maxillofacial surgery at Govt Dental College Srinagar (India). The patients' complaints were limited mouth opening and TMJ pain. Arthrocentesis was performed under aseptic conditions. Clinica… Show more

“…Lately, arthrocentesis has seemed to be more effective in treating pain [5], and some studies have also showed that its cost-effectiveness has been superior to the more conservative approaches [19]. In the present study, there was a significant reduction (p<0.0001) in the perception of pain after arthrocentesis, with VAS values reducing from 7.20±1.37 to 0.43±0.44, being these values similar to the ones found in other studies, from 8.7±1.1 to 1.13±1.16 [6] and from 6.45±1.17 to 1.12±0.42 [20].…”

“…Lavage of the upper joint space reduces pain by removing inflammation mediators from the joint , increasing mandibular mobility by removing intra-articular adhesions, eliminating the negative pressure within the joint, recovering disc and fossa space, and improving disc mobility, which reduces the mechanical obstruction caused by the anterior position of the disc .the results of arthrocentesis were in agreement with studies. [20,24,25] Analysing results of clicking sound in study groups indicated beneficial effect of both treatment modalities in reducing clicking with no statistical significant difference between them. The effect of arthrocentesis in on the clicking sound has been reported in a lot of publication to reduce joint pain, improve function, and reduce clicking as the arthrocentesis is considered an evident method to treat TMJ disc derangement .…”

“…They stated that through the medial joint capsule perforation following the extravasation of fluid was the main reason of edema ( 41 ). Nerve injuries mostly involve facial nerve injury (0.7–0.6 %), trigeminal nerve injury (0.1–2.4 %), otic injury (0.5–8.6 %) ( 42 ). Also, transient facial paralysis as a consequence of the local anesthetic infusion and the following alteration of motor function at the side of AC is one of the possible side effects ( 43 ).…”
