Efficacy of tetracyclines in the treatment of acne vulgaris: a review

Abstract: There is insufficient evidence to support one tetracycline rather than another in terms of efficacy. In the range of investigated dosages, the antibiotic dosage seems to have no impact on efficacy. Despite increased resistance to antibiotics, oral tetracycline formulations displayed no change in efficacy during the study period. Further studies are, however, required to determine if the anti-inflammatory properties of tetracyclines are sufficient in managing acne.

“…Common side effects of tetracyclines are photosensitivity, gastrointestinal complaints, and irreversible dental staining in children [36,38]. Tetracyclines should not be taken together with iron supplements, antacids, or milk, because together they can form insoluble complexes in the intestine, and reduce their absorption.…”

“…Apart from benzoyl peroxide, the use of topical antibacterials such as erythromycin and clindamycin in monotherapy resulted in the development of antibiotic-resistant strains of not only C. acnes but also of S. epidermidis in 4–6 weeks [30, 57, 66]. Systemic antibiotics have been reported to cause only little resistance but can lead to collateral damage in gut microbiota [30, 33, 67]. Oral isotretinoin normalises the response of the innate immune system to C. acnes by inhibiting its proliferation [68].…”

“…77 A systematic review evaluating data from available trials to compare the efficacy of various tetracyclines in the treatment of acne concluded there was insufficient evidence to support one tetracycline relative to another. 78 Tetracyclines are approved by the FDA for short-term treatment of acne in addition to adjunctive therapy.…”
