Treatment of third-class malocclusions often presents a challenge for orthodontists. Skeletal disharmony is often associated with dental malposition. There are several therapeutic choices, including the use in combination of transverse expansion of the maxilla with rapid palatal expander (RPE) and posterior-anterior traction with a Delaire face mask (FM). The purpose of the study is to verify whether there are significant differences in the treatment outcome in the case of use of a face mask followed by a palatal expander or with the sequence of these auxiliaries reversed. Subject and Methods: The two groups were both made up of 13 patients, subdivided into group A, i.e., those whose sequence involved the use of extraoral traction first and then the disjunctor, and those with an inverted sequence in group B. Some cephalometric parameters and dento-skeletal characteristics were evaluated pre-treatment (t0) and at the end of therapy (t1). Results: Considering the T1–T0 of group A (Delaire + rapid palatal expander), the evaluation of the results obtained in this work allows us to observe how within group A there is a significant improvement in the Witts and Nanda indices and facial convexity. Group B (treated with the palate disjunctor sequence followed by traction with Delaire’s mask) showed a significant improvement in ANB, in AoBo, and AppBpp values and in convexity. The two groups were comparable, and no statistically significant difference was highlighted. Discussion: The early therapy of the third skeletal classes by means of a rapid palate expander and face mask is effective. There is no statistically significant difference in the two groups who performed the therapy in reverse mode. This suggests that the clinician should choose the treatment sequence based on the skeletal and occlusal conditions of their patients at the start of treatment. Conclusion: Early therapy of third skeletal classes with sagittal expansion using a rapid palate expander can be performed earlier or later than posterior-anterior traction with a Delaire mask.

Background This study aimed to retrospectively evaluate the treatment efficacy of clear aligners for individuals with mild skeletal Class III malocclusion. Methods A total of 35 patients with non-surgical Class III malocclusion that was treated by clear aligner technology (CAT) underwent analysis using the peer assessment rating (PAR) index, digital model analysis, and X-ray cephalometry. The PAR index was evaluated using plaster casts made before treatment (T0) and at the end of treatment (T1). Using 3DSMax software, a digital model of STL(Standard Template Library) scanned by intraoral scanners at T0 and T1 was reconstructed. Subsequently, an analysis was conducted to compare the distal movement of molars, arch width, and vertical height in three dimensions. X-ray cephalometry analysis was employed to evaluate alterations in the anterior tooth axis and mandibular plane. The significance level was established at P < 0.05. Results Throughout the treatment, the PAR index demonstrated a significant reduction in sagittal variation during the T1 phase, decreasing from 1.85 ± 1.064 to 0.07 ± 0.267. Additionally, the presence of anterior crossbite decreased from 3.33 ± 1.109 to 0.00 ± 0.000, suggesting a complete correction of the anterior crossbite. Digital model analysis indicated that the distal movement of the mandibular molars ranged from 1.7 to 2.0 mm. Additionally, the width of the dental arch in the mandibular premolar area exhibited a significant increase from 32.33 mm to 33.76 mm post-treatment. The SNB (°) decreased from 82.16 ± 3.91 to 80.89 ± 2.41, while the ANB (°) significantly increased from − 1.25 ± 2.12 to -0.07 ± 2.48. The MP-SN plane angle remained relatively stable, changing from 31.50 ± 9.77 to 31.83 ± 5.79 in X-ray cephalometric analysis. Conclusions CAT can effectively address non-surgical Class III malocclusion. In the sagittal plane, CAT can achieve an average of 1.7–2.0 mm distal movement of mandibular molars and demonstrates effective control in both horizontal and vertical directions.

The aim of this study was to investigate the bone area in the posterior region, which is important for mandibular molar distalization in skeletal Class I and Class III individuals with normodivergent and hyperdivergent vertical growth patterns. Materials and MethodsIn this study, cone-beam computed tomography scans of 120 individuals divided into 4 groups as Class I normodivergent (Group-I), Class I hyperdivergent (Group-II), Class III normodivergent (Group-III) and Class III hyperdivergent (Group-IV). Retromolar space at crown level measurements were performed on CBCTderived panoramic radiographs and axial sections. Retromolar space at the root level was measured 2mm, 4mm, 6mm, 8mm and 10mm apical to cemento-enamel junction at CBCT axial sections. ResultsRetromolar space decreased towards the root apex in all groups and smallest retromolar space was level of CEJ 10mm in all groups. At any root level, the distal root of the mandibular second molar tooth was in contact with the mandibular inner or outer lingual cortex; 50% in Group-I, 46.7% in Group-II, 7% in Group-III and 7% in Group-IV. ConclusionsIn normodivergent individuals, the retromolar area length at the root level is observed to be higher in Class III than in Class I at almost every level. In hyperdivergent individuals, on the other hand, only at CEJ 10 level, it is higher in length in Class III than in Class I. Vertical growth pattern has no effect on the root and crown level retromolar space in Class I and Class III individuals. Clinical RelevanceCBCT provides more useful information than panoramic radiographs for patients who are scheduled for large mandibular molar distalization
