Background The number of patients with one or more chronic conditions is increasing globally. One strategy to achieve more sustainable care for these patients is by implementing use of home-based eHealth applications. Such services support patients to take on a more active role as value-creating co-producers of their own care, in collaboration with health care professionals. Health care professionals have a key role in the value creation process, but little is known about value formation within eHealth interactions, especially from their perspective. Therefore, this study aimed to provide a deeper understanding of how an eHealth application can function as a value-creating resource from the perspective of health care professionals. Methods Semi-structured interviews were conducted with thirteen health care professionals (nurses, physicians and first-line managers). Qualitative content analysis was used to analyze the interviews. Results The findings indicate that value formation processes are strongly influenced by the organizational preconditions and by the usability and functionality of technology. The experiences of the health care professionals indicated that value was conceptualized in dimensions of meaningfulness, building of relationships, building safety and feelings of trust. Although these dimensions were mainly expressed in a positive way, such as perceived improvement of medical care, accessibility and continuity, they also had a negative side that caused value destruction. This was primarily due to patient difficulties in using the application or making measurements. Subsequent efforts at value recovery resulted in value creation, but were often time-consuming for the professionals. Conclusions This study contributes by extending conceptualizations of value to the role of health care professionals and by highlighting technology as sometimes facilitating and sometimes hampering value formation processes. The findings indicate that the eHealth application was a value-creating resource, facilitating proactive communication and supporting patients’ engagement and control over their self-care. However, for the application to become a more valuable resource in practice and counteract inequity in care, it needs to be further developed to be adapted to the needs and preconditions of patients.

Child and adolescent overweight is a growing public health problem globally. Europe and low and middle-income (LMIC) countries in Sub-Saharan Africa provide sufficiently suitable populations to learn from with respect to the potential for mobile health (mHealth) interventions in this area of research. The aim of this paper is to identify mHealth interventions on prevention and treatment of childhood and adolescent obesity in Sub-Saharan Africa and Sweden and report on their effects, in order to inform future research in this area. A search of peer-reviewed publications was performed using PubMed, ScienceDirect, EBSCOhost, and Scopus. The search included all articles published up to August 2019. The search strings consisted of MeSH terms related to mHealth, overweight or obesity, children, adolescents or youth and individual countries in Europe and Sub-Saharan Africa. Second, a combination of free-text words; mobile phone, physical activity, exercise, diet, weight, BMI, and healthy eating was also used. Seven studies were reported from Europe and no eligible studies from Sub-Saharan Africa. The results of this narrative review indicate a lack of research in the development and testing of mHealth interventions for childhood and adolescent obesity. There is a need for an evidence base of mHealth interventions that are both relevant and appropriate in order to stem the epidemic of overweight and obesity among children and adolescents in these countries. Uptake of such interventions is likely to be high as there is high penetrance of mobile phone technology amongst adolescents, even within poor communities in Africa.

Міністерство охорони здоров'я України разом із громадськими та державними організаціями, активною медичною спільнотою та ІТ-бізнесом задекларували бажання започаткувати eHealth в Україні вже у 2017 р., як основну компоненту реформ Міністерства. Розглянуто моделі розвитку eHealth в Україні та надано інформацію про основні медичні інформаційні системи, які на сьогодні використовують в Україні. Визначено переваги та недоліки централізованої та децентралізованої систем розвитку eHealth в Україні. Розглянуто основні характеристики сучасних медичних інформаційних систем, які потрібно враховувати у загальному виборі системи. Проаналізовано переваги та недоліки різних підходів, з точки зору реалізації на практиці, та запропоновано рекомендації щодо забезпечення інтероперабельності для галузі охорони здоров'я України. З'ясовано основні характеристики медичної інформаційної системи, на основі яких проведено порівняльний аналіз розглянутих систем. Оцінено вплив різних підходів до забезпечення інтероперабельності в галузі, як результат-запропоновано узагальнену модель досягнення інтероперабельності, яка може бути рекомендована до застосування у вітчизняній системі охорони здоров'я. Ключові слова: інтероперабельність; модель інтероперабельності; електронна система; стандарти; медична реформа України; електронна карта пацієнта.
