Background: The importance of examining the status of gastric inflammation has been acknowledged; the new classification by the updated Sydney system (USS) allows us to evaluate the quantitative status of gastric inflammation. However, this system is based on the histological classification derived from biopsy specimens. Therefore, more convenient, objective and practical biomarkers are recommended. Aim: We undertook a systematic review to find out potential biomarkers by summarizing the relationship between biomarkers and grades of inflammation. Methods: By a primary search via Medline up to December 2004, we extracted a total of 6,526 papers that described biomarkers for human gastric inflammation. All papers were screened by title and abstract. The authors then retrieved 435 citations for complete review; ultimately 231 were appropriate for inclusion criteria. These papers were subclassified into several categories and summarized by each author. Results: In addition to standard markers such as pepsinogens, we confirmed the close relationship between the levels of several biomarkers including gastrin, interleukin-8, HLA class II molecules, reactive oxygen species, and the histological grades. Conclusions: This review provides valuable information describing the clinical implication of these biomarkers for evaluating the static conditions or dynamic alterations of human gastric inflammation.

Summary Leukotriene C4 is an important mediator in the development of inflammatory reactions and ischaemia. Previous studies have shown that leukotriene C4 is able to modulate the function of dendritic cells (DCs) and induce their chemotaxis from skin to lymph node. In this study, we decided to evaluate the modulation exerted by leukotriene C4 on DCs, depending on their status of activation. We showed for the first time that leukotriene C4 stimulates endocytosis both in immature and lipopolysaccharide (LPS) ‐activated DCs. Moreover, it suppressed the interleukin‐12p70 (IL‐12p70) release, but induces the secretion of IL‐23 by DCs activated with LPS and promotes the expansion of T helper type 17 (Th17) lymphocytes. Furthermore, blocking the release of IL‐23 reduced the percentages of CD4+ T cells producing IL‐17 in a mixed lymphocyte reaction. Ours results suggest that leukotriene C4 interferes with the complete maturation of inflammatory DCs in terms of phenotype and antigen uptake, while favouring the release of IL‐23, the main cytokine involved in the maintenance of the Th17 profile.

4Recent observations had suggested a possible Background and objective: Chronic urticaria is one of the most frequent skin diseases and still its etiology is recognized only in a minority of cases. Some recent studies point out to infections due to Helicobacter Pylori as being of major importance in the pathogenesis of chronic urticaria. This study aimed to find out the association of chronic urticaria with H. pylori. Methods: A case-control study was conducted in Erbil city within the period of April 1 st , 2013 to January 1 st , 2014. The study included 55 cases with chronic urticaria and 55 controls that were free from features of chronic urticaria. Data was collected through direct interview and the results of laboratory investigations were recorded in a specially designed questionnaire. Enzyme-linked immunosorbent assay test was used for detection of Helicobacter pylori antigen in the stool sample. Results: The age of the 55 cases and 55 controls enrolled ranged from 13 to 65 years. Stool for Helicobacter pylori antigen test was positive in 69.1% of cases and 29.1% of controls (OR = 5.44, P ˂0.001). The mean age ± SD of positive Helicobacter pylori patients were 35.75 ± 12.64 years, with male to female ratio 1:2.8. No statistically significant association was found between Helicobacter Pylori infection with dyspepsia and duration of urticaria. Conclusions: There was a strong association of chronic urticaria with Helicobacter pylori infection. Investigating for Helicobacter pylori in all cases of chronic urticaria and conducting further trials on Helicobacter pylori eradication is recommended.
