Plasma levels of diazepam and N-desmethyldiazepam were investigated in 19 children by a gas chromatographic method permitting the use of capillary sample. Intravenous administration was studied in 3 children and the plasma level curves showed a rapid decline during the first hour. Absorption and elimination after rectal administration of a solution in 16 children were similar to those after intramuscular administration. Diazepam given by suppository to 5 children gave much lower plasma levels and delayed time to peak levels. Recurrence of seizures in 2 children indicated that the anticonvulsants plasma level was of the order of 150 to 200 mug/liter. No significant side effects were observed. Thus rectal administration of a solution of diazepam is a practical method to arrest convulsions in children.

Summary Forty‐one children with prolonged convulsions were treated with diazepam. Seizures were arrested in 29 patients (71%), but recurred within 12 hr in 14. Respiratory arrest occurred in two patients after intravenous diazepam but in no patient after intramuscular injection. Plasma levels of diazepam and N‐desmethyldiazepam in three children showed that diazepam declines rapidly after intravenous injection; levels above 130 ng/ml were necessary to keep one patient free of seizures. Resumé Quarante et un enfants présentant des convulsions prolongees ont été traités par le diazepam. Les crises ont été arrétées chez 29 patients (71%) mais ont recommencé en 12 H chez 14. Un arrêt respiratoire s'est produit chez deux sujets apres L'injection IV de diazepam, pas aprés 1'injection IM. Le taux de diazepam dans le plasma et du N‐desmethyldiazepam chez trois sujets montrait que le diazepam diminue rapidement aprés 1'injection IV. Des taux supérieurs à 130 ng/ml étaient nécessaires pour maintenir un patient sans crises. ZUSAMMEnfassung 41 Kinder mit anhaltenden Konvulsionen wurden mit Diazepam behandelt. Die Anfälle sistierten bei 29 (71%), rezidivierten jedoch bei 14 Patienten innerhalb 12 Stunden. Bei 2 Patienten ereignete sich ein Atemstillstand nach intravenöser Gabe von Diazepam. Die Plasmaspiegel von Diazepam und N‐desmethyldiazepam bei 3 Kindern zeigten einen schnellen Abfall des Diazepam nach intravenöser Injektion; Spiegel oberhalb 130 mg/ml waren notwendig, um Anfallsfreiheit eines Patienten zu gewährleisten. RESUMEN Cuarenta y un niños con convulsiones prolongadas han sido tratados con Diazepan. Los ataques se interrumpieron en 29 enfermos (71%), pero reaparecieron en 14 enfermos en las primeras 24 horas. Parada respiratoria apreció en dos enfermos después de la administratión de Diazepan intravenoso. Niveles plasmáticos de Diazepan y N‐desmetildiazepan en tres niños revelaron que el Diazepan desaparece rapidamente después de la inyeccion intravenosa; para mantener uno de los enfermos sin ataques se necesitaron niveles por encima de 130 ng/ml.
