Elastase, α1‐proteinase inhibitor, and interleukin‐8 in pre‐dialyzed and hemodialyzed patients with chronic kidney disease

Abstract: The data indicate that in CKD patients, neutrophils are highly activated both in the pre-dialyzed period and on regular HD. Contact with the dialysis membrane during HD causes a significant increase in blood NE-α(1) PI and α(1)-PI in adults, but not in children/young adults. NE-α(1) PI seems to be a much better indicator of an inflammatory state in CKD patients than free α(1)-PI or IL-8.

“…In CKD, plasma elastase levels are elevated, [39][40][41] and primed neutrophils are associated with inflammation and oxidative stress. 28 Our findings indicate that regular exercise does not exacerbate the activation state of resting neutrophils (although transient spontaneous neutrophil activation occurs during each exercise bout).…”

“…The patients with autoimmune disease had complete clinical and biochemical remission. Additionally, for comparative purposes the previously published conservatively treated (CT) group was included (Polańska et al 2010). The CT group consisted of 13 patients, 6 females (46 %) and 7 males (54 %), aged 4–17 years (mean 12 ± 4.5 years), The causes of CKD in the CT group were: chronic glomerular nephritis ( n  = 4), hydronephrosis ( n  = 1), polycystic kidney disease ( n  = 4), kidney hypoplasia ( n  = 1), chronic interstitial nephritis ( n  = 1), and posterior urethral valves ( n  = 2).…”

“…The ability of NE to inactivate the complement components C3 and C5a, immunoglobulin, proteinase inhibitors, clotting factors and adhesion molecules have a direct impact on the course of inflammatory reactions [9]. Secondly, the concentration of neutrophil elastase forming a complex with α 1 -proteinase inhibitor (NE-α 1 PI) correlates with the released free NE elastase and can be used as a measure for the activity of neutrophils during an inflammatory response in different diseases [15][16][17][18].…”
