Electro‐Clinical Investigations on the Role of Proprioceptive Stimuli in the Onset and Arrest of Convulsive Epileptic Paroxysms

Abstract: SUMMARY A study was carried out on two cases of reflex epilepsy with hemiconvulsive seizures induced by proprioceptive stimulations (tuning fork vibrations and active and/or passive movements). A proprioceptive dissociation is shown to exist in reflex epilepsy, i.e. the seizures are induced only by certain proprioceptive stimuli (vibration or movement). It is assumed that the proprioceptive stimuli generated during the convulsive seizure intervene by a feedback mechanism. By means of this feedback mechanism th… Show more

“…The proprioceptive input serves as an important afferent to the postcentral area in PLE, triggering the spread of seizures. 8 However, one important challenge of proprioceptvie-induced seizures is the differential from paroxysmal movement disorders. The proprioceptive triggers in epileptic patients can be described as passive or active movement of the extremities, 8,9 misleading clinicians to other diagnosis such as paroxysmal kinesigneic dyskinesia (PKD).…”

“…8 However, 1 important challenge of proprioceptive-induced seizures is the differential from paroxysmal movement disorders. The proprioceptive triggers in patients with epilepsy can be described as passive or active movement of the extremities, 8,9 misleading clinicians to other diagnosis such as PKD. Key points helpful for the differentiation between PLE and PKD are summarized in the Table . In particular, careful investigation of the specific triggers and description of each attack are important.…”

“…Epileptic nature of the attacks is shown by the ictal EEG recordings. 75 The maximum EEG electronegativity is seen at the central vertex electrode in the seizures induced by walking. 76 Most of patients respond properly to insulin therapy in non-ketotic hyperglycemia and only a few of them need AEDs where carbamazepine and clobazam can be used in the patients requiring treatment.…”

“…The effects of sensory stimuli, mental activity and emotional state have been studied on generalized discharges (Symonds 1959;Jung 1962;Paulson 1963;Arseni et al 1967) and on petit ma1 absence (Jung 1962; Ounsted and Hutt 1964;Vidart and Geier 1967;Guey et al 1969). There are few studies of interictal focal activity.…”
