Electron microscopic analysis of sequential liver biopsy samples from patients with rheumatoid arthritis. Correlation with light microscopic findings

Abstract: We used electron microscopy (EM) to analyze 52 biopsy samples from 22 patients who were receiving long-term weekly oral doses of methotrexate (MTX) for the treatment of rheumatoid arthritis. Forty-eight biopsy samples were obtained after 2-6 years of continuous treatment, and 4 samples were obtained before treatment was begun. Specimens were graded for neutral fat, secondary and tertiary Iysosomes, and smooth endoplasmic reticulum (SER) in hepatocytes, and for collagen in the perisinusoidal space (Disse's spac… Show more

“…We have previously reported on light and electron microscopic analysis of hepatic biopsy specimens over a mean period of -3.5 years of MTX therapy (1 1, 14). The present report expands and extends our observations on the same patients, who have continued to undergo regular sequential liver biopsies through a mean followup period of 8.2 years.…”

“…Transient elevations of liver enzymes occur frequently, but their long term significance is unclear, since they do not correlate with histological abnormalities. Moreover, sequential liver biopsies in patients continuing methotrexate therapy showed no evidence of progression of abnormalities in hepatic architecture (Kremer et al 1991;Sawitzke et al 1991;Scully et al 1991). “…This consists of progressive hypoxaemia associated with interstitial and alveolar infiltrates and varying degrees of bronchiolitis and fibrosis [66,67]. Hepatic fibrosis may occur with low dose methotrexate therapy, but it is rare for this to have any significant clinical effect [68].…”
