Electrothermal bipolar vessel sealing system in axillary dissection: A prospective randomized clinical study

Abstract: We assessed whether axillary dissection using the electrothermal bipolar vessel sealing system (LigaSure) improved perioperative outcome when compared with conventional axillary dissection, in a prospective randomized study of 100 women with breast cancer. Those needing axillary dissection were randomized to the use of LigaSure or to conventional axillary dissection (with 50 patients in each group, all of whom had a closed suction drain in the axilla). The LigaSure patients had less intraoperative blood loss (… Show more

“…Surgeons should reduce the use of electrocautery in order to reduce seroma formation [3]. New surgical devices, like the electrothermal bipolar vessel sealing system, show promising results but need to be evaluated with a ‘no-drain' control group [40,55,56]. …”

“…e conclusion of one review is that UD does not improve outcome [92], compared to ES; in another review there is a 70% lower risk of axillary seroma formation a er surgery with UD than with ES [93], and in axillary dissection, EVS reduces operating time, days of suction drain, and length of hospital stay, without increasing complication rate, compared with conventional ES [94] Operations in the pelvis e sympathetic nerves derive from T10-L2 and form a series of ganglia consisting of the sympathetic chain just medial to the origin of the psoas muscle ( Figure 10). e bers supplying the bladder, rectum, and genital organs have their origin in the T12-L2 nerve roots, and enter the hypogastric plexus below the aortic bifurcation and form two bundles of bers just medial to the iliac vessels.…”

“…Se ha demostrado que el volumen de débito del drenaje axilar y la aparición de seroma, si el drenaje es retirado de forma precoz, son independientes de la técnica quirúrgica realizada, de los instrumentos de disección y sellado utilizados [5][6][7][8] o de la adición de sellantes de fibrina 9 .…”
