Elemental diet for refractory atopic eczema.

Abstract: A total of 37 children with refractory widespread atopic eczema were treated with an antigen avoidance regimen comprising hospitalisation, exclusive feeding with an elemental formula for a median duration of 30 days, and measures to reduce exposure to pet and dust mite antigens at home. After the initial period of food exclusion, food challenges were performed at intervals of seven days, and the patients followed up for at least 12 months.

“…Dietary treatment has risks: normal toddlers can become picky eaters, compliance with the diet may be a problem (7,19) and restricted diets can be nutritionally inadequate (27,28). Also, in highly sensitive food-allergic patients, restricted diets carry a risk of an anaphylactic reaction if the food is accidentally ingested (22,29). Exclusion diets should not be undertaken lightly, and never without proper supervision by a dietician.…”

“…This test preferentially measures contact urticaria. Dutch workers have found a correlation between SAFT and prick tests in 64 children, although the use of fresh foods gives a higher frequency of reactions (80% vs. 52%) 23 . However, elimination diets do not lead to clearing of the eczema, but to lower disease activity.…”

“…Suitable products are listed in Table 3. These formulae have been used mainly for children with severe eczema [14] and also to obtain remission in Crohn's disease [15]. Any improvements in patients with Crohn's disease may relate to the lack of residue rather than low allergenicity.…”
