ELSO Guidelines for Adult and Pediatric Extracorporeal Membrane Oxygenation Circuits

Abstract: Disclaimer: This guideline describes the ECMO circuit for all patient populations. These guidelines describe safe practice based on extensive experience and are considered consensus guidelines. These guidelines are not intended to define standard of care and are revised at regular intervals as new information, devices, medications, and techniques become available.

“…In particular, many efforts have been made to clarify the applicability and benefits of intraoperative (io) veno-arterial extracorporeal membrane oxygenation (VA ECMO), using it not only as a ‘rescue’ device helpful for managing acute life-threatening complications (i.e., pulmonary hypertension with right ventricular failure) but also as an io extracorporeal support routinely applicable ‘in advance’ for every patient [ 3 , 4 , 5 , 6 ].…”

“…VV-ECMO is indicated for the treatment of patients with any potentially reversible respiratory complication where the lungs are unable to provide sufficient ventilation and oxygenation despite the use of conventional treatment ( 25 ). The main indications of ECMO in respiratory support are divided into: (1) rescue of patients with refractory hypoxemia suffering from inadequate arterial oxygenation despite optimal levels of inspired oxygen, (2) rescue of patients with refractory hypercapnia leading to high CO 2 blood levels, (3) rescue of patients from injurious mechanical ventilation ( 24 ).…”
