Emergence of genetically linked vaccine-originated poliovirus type 2 in the absence of oral polio vaccine, Jerusalem, April to July 2022

Abstract: We report an emergence and increase in poliovirus type 2 detection via routine wastewater surveillance in three non-overlapping regions in the Jerusalem region, Israel, between April and July 2022. Sequencing showed genetic linkage among isolates and accumulation of mutations over time, with two isolates defined as vaccine-derived polioviruses (VDPV). This demonstrates the emergence and potential circulation of type 2 VDPV in a high-income country with high vaccine coverage and underscores the importance of ro… Show more

“…In the model, if any OPV2‐related virus reaches high transmission settings, like some areas of India and Bangladesh, very high type 2 case counts would likely follow. Recent importations and transmission of cVDPV2s in high‐income countries, including Israel (Zuckerman et al., 2022), the United Kingdom (United Kingdom Department of Health & Social Care, 2022), and the United States (Link‐Gelles et al., 2022; Ryerson et al., 2022), confirm that cVDPV transmission can occur even in countries with high overall reported IPV immunization coverage in communities with low coverage (Thompson et al., 2012), with paralytic cases possible in these communities. Countries with relatively lower immunization coverage should expect to fair worse with respect to potential case counts, and they should recognize the need for large and high‐quality outbreak responses if they want to keep case counts lower.…”

“…Even in countries with very high IPV‐only coverage, imported live polioviruses may sustain some transmission in populations and potentially cause paralytic cases in undervaccinated subpopulations (Duintjer Tebbens et al., 2005; Kalkowska, Duintjer Tebbens, Grotto, et al., 2015; Thompson et al., 2012, 2015). Recent detections of polio transmission in the United Kingdom (United Kingdom Department of Health & Social Care, 2022; Israel Zuckerman et al., 2022), and the United States (Link‐Gelles et al., 2022; Ryerson et al., 2022) in un‐ or underimmunized populations provide proof of concept. Thus, in the event that OPV use continues, countries will need to consider their national immunization policies and decide which vaccines and how many doses to deliver as part of RI.…”

“…Wastewater samples from 48 sewersheds in Rockland and surrounding counties were then tested for poliovirus from 9 March to 11 October 2022, with 8.3% of the 1,076 samples (from 10 sewersheds) having detectable concentrations of the virus (Ryerson, 2022). Routine wastewater surveillance (April to July 2022) in Jerusalem, Israel detected and identified an increase of vaccine-originated poliovirus type 2 even in the absence of oral polio vaccine (Zuckerman et al, 2022).…”
