IntroductionParamedicine is a domain of practice and health profession that specialises in the provision of health and social care across a range of settings including, but not limited to, emergency and primary care. Paramedics work in a variety of clinical settings such as paramedics services, hospitals, and clinics, in the community as well as non-clinical roles, such as education, leadership, policy work, public health and research. A previous competency framework for paramedics in Canada had become increasingly poorly aligned with contemporary practice. We sought to develop a competency framework that represents contemporary paramedic practice in Canada. MethodsThis project was guided by a conceptual framework informed by systems thinking and used a six-step model for developing competency frameworks. We engaged paramedics from across Canada in working groups to explore various contexts of paramedic practice. We gathered data using multiple methods and coded this data into activity statements. We grouped these statements and developed or expanded corresponding professional activities through five drafts of the framework from 2022 to 2024. A technical committee reviewed and edited the draft framework throughout the process, culminating in a public review period from January to March 2024, and a subsequent final version of the National Competency Framework for Paramedics (NCFP). ResultsThe National Competency Framework for Paramedics reflects contemporary paramedic practice in Canada. It describes the professional activities of paramedics organised under five domains - Person-Centred Care; Collaborative Care; Safe Care; Self-Care; and Professional Care. Paramedics perform these activities across multiple contexts of practice by enacting a variety of roles depending on the event. Discussion and conclusion By exploring the variety of practice contexts and the expectations of paramedics across Canada, the NCFP provides a holistic, contemporary description of paramedic practice in Canada, providing a foundation for professional practice in a variety of settings. The five domains provide a comprehensive representation of paramedic practice helping to ensure care is ultimately and sufficiently aligned with the needs of the community and settings in which paramedics can practice. The NCFP can be used to inform education programs to better prepare paramedics for complex, interprofessional health and social care practice now and as practice continues to evolve.

Health and social needs exist along a dynamic continuum. Recognising that health status is inextricably impacted by social determinants of health, community paramedicine has opportunities and a responsibility to reduce inequities. The objective of this scoping review was to investigate the peer‐reviewed and grey literature to explore how community paramedicine supports community needs along a health and social continuum. We conducted a scoping review of the English language literature using the JBI Scoping Review methodology. We searched CINAHL, EMBASE, MEDLINE, Google Scholar, and organisational websites. 30 peer‐reviewed and 13 grey literature articles met the inclusion criteria. The findings describe the ways community paramedicine models evolved from minimising system pressures on emergency health services to addressing health and social needs. A key recommendation across the literature was the need to meaningfully engage communities early in the program development to understand how best to implement and codesign integrated service models addressing specific community needs although there was a lack of evidence to guide this approach. There is a notable lack of evidence pertaining to optimising technologies in the program design and implementation. The results highlight opportunities to determine the best practices for conducting holistic community needs assessments that include equitable stakeholder engagement and enhancing education to prepare paramedics for expanded roles. Community paramedicine provides opportunities to better meet the needs of structurally marginalised communities. There is a social responsibility and opportunity to engage communities to codesign service delivery, advance paramedic education, and enhance interprofessional collaboration to better support community needs and generate upstream solutions for individuals and communities.

A key component to the continued development of any profession is a strong research culture. For paramedicine, a research culture that acknowledges the uniqueness of the out-of-hospital setting allows clinical practice to develop. Recent international publications investigating paramedicine research outputs demonstrated that Ireland was not strongly represented in the production of paramedicine-based research. The aim of this scoping review was to identify and evaluate the quantity and quality of the research published to date in the Irish out-of-hospital literature, and to identify research gaps that may need to be addressed. A scoping review was conducted using the Joanna Briggs Institute guidance. All studies that reported on out-of-hospital care in the Republic of Ireland were included. MEDLINE, CINAHL, EMBASE, Cochrane Library, and grey literature sources were searched from 2000 until 2022 for relevant articles in March 2023. Key authors were contacted to identify missing articles and data were extracted to present the final publication yield. Scientific quality was assessed using Clarivate Average Journal Impact Factor Percentiles. Over 3200 research outputs were identified from the databases with a final yield of 294 research outputs included in this review. Research outputs were identified for most years included in the review period, with outputs published in a large range of journals. Research heavily focused on areas linked to the Clinical Practice Guidelines, and predominantly used quantitative data collection methods. Most authors identified in this review were named on only one output. However, diversification in research topics and methodologies, a concerted effort to ensure practitioners are leading research and support to ensure that conference presentations are continued to peer-reviewed publication would strengthen this resolve.
