Emotional and Personality Traits are Determinants of Activity Avoidance in Chronic Rhinosinusitis Patients

Abstract: Objectives: Chronic rhinosinusitis (CRS), like other sinonasal diseases, may be associated with avoidance of daily activities. Our goal was to identify characteristics associated with avoidance of activities due to CRS. Materials and Methods: A total of 194 CRS patients were recruited. CRS symptom burden was assessed with the 22-item Sinonasal Outcome Test (SNOT-22). SNOT-22 nasal, sleep, ear/facial discomfort and emotional/psychological subdomain scores were calculated. Depressed mood was assessed using the t… Show more

“…These include chronic sinonasal symptomatology 20,21 and associated exacerbations [22][23][24] which require systemic antibiotics or corticosteroids, [25][26][27][28] exacerbations of comorbid pulmonary disease, [29][30][31][32][33] and social consequences such as missed work or activity avoidance. [34][35][36][37] Sinonasal symptoms represent the dominant burden experienced by patients with CRS. 20,21 CRS disease control may be assessed based on sinonasal symptoms, the need for systemic antibiotics and corticosteroids, and the objective burden of disease on endoscopy.…”

“…6 Depressed mood and emotional disturbance due to CRS have also been shown to be a major determinant of missed work and school as well as activity avoidance. 7,8 Moreover, depression and anxiety are associated with worse QOL outcomes after treatment for CRS. [9][10][11] Depression and GAD in patients with CRS therefore represent 2 clinical challenges.…”

“…Depression and anxiety have been found to worsen the patient‐perceived burden of disease 6 . Depressed mood and emotional disturbance due to CRS have also been shown to be a major determinant of missed work and school as well as activity avoidance 7 , 8 . Moreover, depression and anxiety are associated with worse QOL outcomes after treatment for CRS 9–11 .…”

“…Previously described, validated metrics of disease control for asthma and allergic rhinitis have included a patient-reported assessment of overall disease control as a means for direct patient input in relation to the control construct that is being measured (15)(16)(17)(18) . Because disease control is likely a complex construct, which may vary in its composition from patient to patient, and CRS has many manifestations including its associated symptomatology (19,20) , exacerbations (21)(22)(23) which require systemic antibiotics or corticosteroids (24)(25)(26)(27) , exacerbations of comorbid pulmonary disease (28)(29)(30)(31)(32) , and social consequences such as missed work or activity avoidance, patient input on assessment of control is paramount (33)(34)(35)(36) . A direct assessment of patient-reported CRS disease control that could potentially capture patients' internal synthesis of disease manifestations to form their overall disease experience may therefore serve a valuable role in a global assessment of CRS disease control.…”
