Emphysematous Cystitis

Abstract: Emphysematous cystitis is a relatively rare and potentially life-threatening condition characterized by the collection of gas in the bladder wall and lumen due to infection caused by gas-forming organisms. Imaging studies are necessary to detect emphysematous cystitis. The management consists of broad-spectrum antibiotics, strict glycemic control, and bladder drainage. Complications may arise in some cases, requiring surgical treatment. We present a case of extended spectrum beta-lactamase producing Escherichi… Show more

“…However, pyocystis is unlikely in this case because it has been reported in oliguric patients, such as patients with supravesical urinary diversion or hemodialysis-dependent patients [14] . Although there is no report of emphysematous cystitis due to S. salivarius infection [1] , [2] , [15] , [16] , S. salivarius can lead to gas formation. The urease system in S. salivarius breaks down urea to ammonia and CO 2 [17] .…”

“…The mean age of emphysematous cystitis occurrence is approximately 66 years, and it occurs twice as often in women than in men [1] . Diabetes mellitus and urinary stasis are well-recognized risk factors [2] . In emphysematous cystitis, Escherichia coli and Klebsiella pneumoniae are often isolated from urine cultures [1] .…”

“…Age older than 60 accounted for three-quarters of EC patients, and females were the predominant gender with an incidence of 63.7–65.5% [ 2 , 3 ]. Therefore, we considered older age to contribute to the higher mortality rate [ 2 , 3 , 15 , 16 , 17 ]. In addition, risk factors in EC have been thoroughly investigated, including DM, recurrent UTI, neurogenic bladder (NB), bladder outlet obstruction, blood supply disorders, and prolonged catheterization [ 2 , 14 , 15 , 16 ].…”

“…Macrophages are found in the normal kidney and in increased numbers in the diseased kidney, where they act as key players in kidney injury, inflammation, and fibrosis [10]. Classically activated macrophages (M1 macrophages) and alternatively activated macrophages (M2 macrophages) are two main phenotypes [11]. M2 macrophages have anti-inflammatory functions and are involved in parasite containment, wound healing, and fibrosis, while M1 macrophages exert pro-inflammatory effects, mediate antimicrobial defense, and antitumor immunity [12].…”
