“Empirical Treatment in Patients with Acute Obstructive Pyelonephritis”

Abstract: Acute obstructive pyelonephritis (AOP) is a urological life-threatening inflammatory condition that initially requires immediate urinary drainage and simultaneous prescribing of highly effective targeted antibacterial treatment. The empirical antibacterial therapy is always prescribed before urine culture and susceptibility testing are done. Percutaneous nephrostomy (PNS) and ureteral stenting (US) are the two options for urinary drainage in obstructed kidneys, while fluoroquinolones and 3(rd) generation cepha… Show more

“…This is explained by the existence of bacteria with mechanisms of adherence to foreign materials. Broad-spectrum antibiotics were used due to the high rate of antibiotic resistance, as described in other articles [ 19 ]. In patients who underwent extrusion (53–83 months), this was due to a chronic bypass infection and presented a positive urinary culture to bacteria highly adherent to the prosthetic material.…”

“…However, when urosepsis is suspected or known to be present, preprocedural antibiotics are recommended [17,18,37,43]. A recent study demonstrates superiority of thirdgeneration cephalosporin ceftazidime versus fluoroquinolone ciprofloxacin in both clinical and microbiological cure rates with improved early and long-term cure rates in those who received PCN versus ureteral stent [77].…”
