Endobronchial Metastasis from Renal Cell Carcinoma as a Reason for Recurrent Pulmonary Infections

Abstract: Endobronchial metastases (EBM) secondary to extrathoracic malignancies are very rare. Breast cancer, colorectal cancer and renal cell carcinoma represent the most common types of cancer leading to endobronchial metastases. They usually represent a late manifestation of other types of cancer and their prognosis is generally poor averaging a survival of 1-2 years in most case series. Due to their rarity, they remain a challenge for clinicians regarding whether they are primary lung tumors or not. This case repor… Show more

“…The most common symptoms mentioned are cough and hemoptysis followed by dyspnea and wheezing in patients with EBM. [ 10 11 12 13 14 15 16 ] In our series, cough was the most frequent symptom followed by chest pain and dyspnea. This indicates that if there is any one of these symptoms in a patient with extrapulmonary malignancy, especially breast, colon or renal tumors, bronchoscopic examination is must to exclude EBM.…”

“…According to the literature, EBM is observed in various types of malignancies, including colorectal, breast, kidney, stomach, ovarian, thyroid, uterine, testicular, nasopharynx, prostate, adrenal carcinomas, sarcomas, histiocytoma, and plasmacytomas. [ 10 11 12 13 14 15 16 ] The most common EBM is from carcinomas of the colorectal region followed by the breast and kidney. [ 13 14 15 ] This was very well correlated with our study as carcinoma breast and colon were the most common.…”

“…When surgery is not possible, chemotherapy and external radiotherapy may be considered, especially in localized occluding lesions. However, they are limited of efficacy in RCC metastases, as they do not respond well to such modalities [ 21 , 22 ].…”

“…Many palliative modalities can be also contemplated in the late stages of the tumor progression. These modalities include endobronchial therapies such as cryotherapy, laser therapy, brachytherapy, diathermic snares, intratumoral ethanol injections, argon plasma coagulation (APC), and stents [ 4 , 22 ].…”

“…Those with advanced esophageal cancer, complicated by an obstruction, have an increased the risk of aspiration pneumonia. Endobronchial lung tumors are associated with chronic inflammation and recurrent post-obstructive infections [16]. Intra-abdominal tumors obstructing the genitourinary or hepato-biliary tracts may result in severe urinary tract infections or cholangitis [17,18].…”
