Abstract:This report discusses the case of a failed root canal treatment in the right mandibular first molar, which exhibited a chronic periapical pathology in its distal and mesial roots. The tooth was managed surgically by periapical curettage and apicoectomy. This approach is indicated in teeth having periapical pathosis which does not resolve by routine endodontic treatment.
