Endometrial ablation plus levonorgestrel releasing intrauterine system versus endometrial ablation alone in women with heavy menstrual bleeding: study protocol of a multicentre randomised controlled trial; MIRA2 trial

Abstract: Background It is estimated that between 12 to 25% of women who undergo an endometrial ablation for heavy menstrual bleeding (HMB) are dissatisfied after two years because of recurrent menstrual bleeding and/or cyclical pelvic pain, with around 15% of these women ultimately having a hysterectomy. The insertion of a levonorgestrel-releasing intrauterine system (LNG-IUS) immediately after endometrial ablation may inactivate residual untreated endometrium and/or inhibit the regeneration of endometr… Show more

“…A multicenter RCT (the MIRA2 study) has recently been started in the Netherlands comparing endometrial ablation plus LNG-IUS with endometrial ablation alone. 75 In this systematic review, all published articles that report hysterectomy incidence after endometrial ablation, with a minimum follow-up of 1 year, are presented. An extensive number of studies was included in this review; therefore, a representative number for hysterectomy rate after endometrial ablation was defined.…”

“…Unfortunately, there were no studies in Jordan that compared these different modalities regarding particularly cost-effectiveness and patient satisfaction. There is an ongoing a multicenter randomized controlled trial to evaluate if the combination of endometrial ablation and an LNG-IUS is superior to endometrial ablation alone in terms of reducing subsequent rates of hysterectomy at two years following the initial ablative procedure [20].…”

“…A systematic review evaluating this combination shows a favourable outcome for the EA plus LNG-IUS (Oderkerk et al, 2021). A RCT needs to be done to elucidate the real advantages and outcome of this combination (Oderkerk et al, 2022).…”
