Primary retroperitoneal serous adenocarcinoma is an extremely rare clinical entity, with only five cases previously reported. We describe a 66-year-old woman with serous adenocarcinoma arising from the retroperitoneum, adhesive to the ileocecum. On MRI, the tumor was 17 x 10 x 6 cm in size and monocystic. The tumor cells were positive for CA125, estrogen receptor, and Wilms tumor 1, and were differentiated from enterogenic cyst by the presence of cytokeratin 7 (+), and the absence of cytokeratin 20 (-). Simultaneously, uterine adnexal serous adenocarcinoma was detected. The volume of the retroperitoneal tumor was 14 times larger than that of the uterine adnexal tumor. We suggest the possibility that the retroperitoneal cancer arose as a primary cancer from rests of the müllerian epithelium synchronously with the uterine adnexal cancer. The patient had been taking conjugated estrogen for 10 years, suggesting that long-term estrogen stimulation may have contributed to the progression of the tumors.

We present the case of a 45-year-old female patient who was admitted with a history of pelvic pain, constipation, and dysmenorrhea. CT scan and u/s images revealed cholelithiasis, benign nodular hyperplasia of segment IV of the liver and uterine fibromyoma. During laparotomy, firm adhesions between the posterior wall of the uterus and the rectum were found and the incisional biopsy reveals an undifferentiated adenocarcinoma. Then, total resection of the uterus was performed with en block resection of the adherent part of the rectum and part of the posterior wall of the vagina. The final histopathological report showed the presence of uterine fibromyoma, nodular hyperplasia of the liver and rectal endometriosis without any sign of malignancy. The patient after 5 years of follow up remains healthy. Rectal endometriosis represents an uncommon localization of pelvic endometriosis where the symptoms and clinical findings are non-specific making the definitive preoperative diagnosis difficult. Endometriosis should be included in the differential diagnosis of chronic pelvic pain in combination with defecation disorders in female patients of reproductive age.

Endometrioid carcinoma developed on an endometriosis of sigmoid mesocolon a case report

Degenerate endometriosis is very rare. It is usually found in the ovaries and is rarely extragonadic. We report on an endometrial carcinoma that developed on a sigmoid mesocolon endometriosis in a 45-years old patient. The tumour was discovered by a CAT scan which was performed because of the existence of pelvic pain. The patient had undergone total hysterectomy with bilateral annexectomy and a biopsy of the tumour. The histology examination showed the tumour to be an endometrial adenocarcinoma whose cells express the anti CK7 antibody and do not express the anti CK20 antibody.Our observation seems very unique in the sense that the symptoms were not directly related to the tumour. Pelvic pain was probably more related to blood collection secondary to cyclical bleeding in endometrial area. Because of this, the tumour was discovered at an early stage and diagnosis was made by part of the tumour being excised and not on a piece of colic resection.Ré sumé : L'endomé triose dé gé né -ré e est trè s rare. Elle siè ge é lectivement au niveau des ovaires et est rarement extragonadique. Nous rapportons l'observation d'un carcinome endomé trioïde dé veloppé sur une endomé triose du mé socô lon sigmoïde chez une patiente de 45 ans. La tumeur a é té ré vé lé e à l'examen tomodensitomé trique ré alisé devant la pré sence d'algies pelviennes. La patiente a subi une hysté rectomie totale avec annexectomie bilaté rale et une biopsie exé rè se du foyer tumoral. À l'examen histologique, la tumeur correspondait à un adé nocarcinome endomé trioïde dont les cellules exprimaient l'anticorps anti-CK7 et n'exprimaient pas l'anticorps anti-CK20. Notre observation semble ê tre unique dans le sens où le signe d'appel n'é tait pas directement lié à la lé sion tumorale. Les douleurs pelviennes ré vé latrices é taient lié es plutô t à la collection sanguine secondaire au saignement cyclique du foyer d'endomé triose. De ce fait, la tumeur est dé couverte à un stade pré coce et le diagnostic s'est fait sur un fragment d'exé rèse de la tumeur et non pas sur une piè ce de ré section colique.
