Endometriosis-Associated Ovarian Cancer: Population Characteristics and Prognosis

Abstract: The presence of coexisting endometriosis did not change the prognosis of EOC but was associated with better OS in patients with CCOC. Patients with CCOC and EOC at initial stages and EOC at advanced stages have a good prognosis; however, CCOC at advanced stages had a sooner recurrence and shorter OS.

“…Relevant articles have reported that approximately 25~80% of patients with EOVC and OCCC also have a diagnosis of endometriosis ( 18 , 19 ). Patients with endometriosis have been reported to be at a 1.49, 3.73 and 2.32 times greater risk of development of OC, OCCC and EOVC respectively, compared with healthy women without endometriosis ( 20 ).…”

“…Recently, an article defined “high-risk” cases as those which share the same mutations present in EOVC and endometriosis, and “low-risk” cases as those not sharing any mutations in endometriosis with the carcinoma ( 30 ). According to the literature, compared with women without endometriosis, women with endometriosis are more likely to be diagnosed with early-stage tumor, with a significantly lower level of serum CA125 before surgery, and are less likely to have lymph node metastasis or to develop platinum resistance disease ( 18 , 31 ). Patients with EOVC arising from endometriosis presented at a lower average age, have a higher percentage of early-stage and lower–grade disease, and are more likely to have no residual disease after primary debulking surgery compared with those without endometriosis, which indicates better survival outcomes ( 32 ).…”

“…Use of tumor markers to distinguish endometriosis-related ovarian neoplasms from ovarian endometrioma IntrOdUCtIOn Endometriosis may be associated with a subset of histologic subtypes of ovarian cancer. [1][2][3] Both ovarian clear cell carcinoma and endometrioid carcinoma are often associated with endometriosis, [4][5][6] sharing similar histogenesis and pathogenesis, 7 and are known as endometriosis-associated ovarian cancer. Endometrioid ovarian carcinomas differ from highgrade serous ovarian cancers at the mutational level.…”

“…Ovarian endometriosis was strongly associated with an increased risk for ovarian cancer. In spite of favorable characteristics of EAOC, the findings indicated that endometriosis did not affect ovarian cancer prognosis after its onset, although a better overall survival was reported, primarily in clear cell carcinoma (30,75,76).…”

“…A second revision by an expert pathologist identified another 11 cases with endometrial foci. From the remaining 50 cases after exclusion criteria, 40 cases (80%) were diagnosed as ovarian cancer associated with endometriosis (75).…”
