Endoscopic Approaches to Obesity

Abstract: Given the emerging role of endoscopic procedures in the treatment of obesity and rapid changes in endoscopic technologies and techniques, this review considers the current state of endoscopic management of obesity. Endoluminal interventions performed entirely through the GI tract by using flexible endoscopy offer the potential for an ambulatory weight loss procedure that may be safer and more cost- effective compared with current surgical approaches. Endoscopic techniques attempt to mimic the anatomic features… Show more

“…Endoluminal sleeves and intragastric balloons have demonstrated short-term efficacy in inducing weight loss as well as concurrent resolution or improvement of obesity-related comorbidities [7]. A newly proposed technique, the laparoscopic fundoplication combined with mediogastric plication, has been reported to offer improvements in gastroesophageal reflux disease with excess weight loss approaching that of RYGB in patients with a BMI of 32–35 kg/m 2 after 1 year, although long-term data is lacking [8].…”

“…Currently, similar to bariatric surgery, there are 2 major approaches to endoscopic bariatric therapies: restrictive and malabsorptive (Figure ). The duodeno‐jejunal bypass sleeve (DJBS) is an example of malabsorptive therapy, whereas the restrictive procedures include adjustable intragastric balloons (IGBs), endoscopic sleeve gastroplasty (ESG), trans‐oral gastroplasty, and trans‐oral endoscopic restrictive implant system . Data are emerging to indicate that these procedures may also have significant benefit for patients with NAFLD.…”

“…Recent surgical trends have sought to achieve a smaller scar and use a minimally invasive approach. Endoluminal bariatric procedures, which are performed using flexible endoscopy completely through the GI tract, offer the potential for an ambulatory weight loss procedure that may be more cost-effective and safer than current surgical bariatric procedures [ 11 ]. Endoscopic bariatric procedures have been attempted as a type of relatively noninvasive approach related to the restriction of food intake or malabsorption.…”
