Background Gastric cancer (GC) is a highly heterogeneous disease with many different histological and molecular subtypes. Due to their reduced systemic adverse effects, nanoformulation agents have attracted increasing attention for use in the treatment of GC patients in the clinic. To improve therapeutic outcomes, it is vitally necessary to provide individual medication references and guidance for use of these nanoformulations, and patient-derived organoids (PDOs) are promising models through which to achieve this goal. Results Using an improved enzymatic digestion process, we succeeded in constructing GC PDOs from surgically resected tumor tissues and endoscopic biopsies from GC patients; these PDOs closely recapitulated the histopathological and genomic features of the corresponding primary tumors. Next, we chose two representative paclitaxel (PTX) nanoformulations for comparative study and found that liposomal PTX outperformed albumin-bound PTX in killing GC PDOs at both the transcriptome and cellular levels. Our results further showed that the different distributions of liposomal PTX and albumin-bound PTX in PDOs played an essential role in the distinct mechanisms through which they kill PDOs. Finally, we constructed patient-derived xenografts model in which we verified the above distinct therapeutic outcomes via an intratumoral administration route. Conclusions This study demonstrates that GC PDOs are reliable tools for predicting nanoformulation efficacy. Graphical Abstract

Five-FU is a potent chemotherapeutic agent for suppressing endothelial cell growth. The purpose of this study was to investigate the usefulness of local peritumor injection of 5-FU for patients with advanced gastric cancer (AGC) for the prevention of anemia. Between January 2020 and January 2022, patients aged 18 years or older with AGC and moderate anemia were included. A total of 200 mg of 5-FU was injected per session at ten points of the lesion (20 mg at each point) every 7 days for 4 to 12 weeks. Patients received a blood test for toxicity at every cycle. From one of these patients, endoscopic biopsy specimens were taken from gastric cancer before and after injecting 5-FU for immunostaining. A total of five AGC patients participated in this study. For most patients, hemoglobin levels were maintained without transfusions during 5-FU injection, and expression levels of thrombospondin-1 was increased after injection compared to those before injection. Blood test results during 5-FU injection showed no significant change in serum glutamic oxalacetic transaminase/glutamic pyruvic transaminase, total bilirubin, or creatinine level. The results of this study showed the possibility of local peritumor 5-FU injection as a treatment for relieving anemia of patients with gastric cancer.

63 patients with advanced gastric cancer were randomly divided into 2 groups: therapy group and control group. Therapy group received carbon nanoparticles loaded with 5-fluorouracil injection in tumor vicinity. The compares of the number, the metastasis degree and black-stained of lymph node in the radical operation between the two groups were studied, and the adverse drug reactions were observed. In the therapy group, the average number of eliminated lymph nodes (43.73) was far higher than that of control group (25.36) (P 0.05). And the dyed lymph nodes were 26.2% which was higher than undyed lumph nodes (4.5%) and that of control group (16.9%) (P 0.05). There were no local and systemic adverse reactions happened after nano-activated carbon loaded with anticancer drug local injection. Local injection of nanometer activated carbon loaded with 5-fluorouracil around the tumor is important to metastasis lymph node resection, especially to the tiny metastasis lymph resection in gastric radical operation. It has lymphatic chemotherapy effect.
