Endoscopic closure devices: A review of technique and application for hemostasis

Abstract: Acute gastrointestinal bleeding is a common, yet challenging, and potentially problematic medical emergency for clinicians to manage. Given its significantly high health care associated costs and increased morbidity and mortality rate, new endoscopic techniques for managing gastrointestinal bleeding has gained considerable attention. While conventional methods (i.e., injection, cautery, and hemostatic clips) have been widely used, their efficacy and reliability for initial hemostasis has been hindered by high … Show more

“…The OverStitch endoscopic suturing system has been reported to cost approximately $1000, with an additional cost of $125 per unit for each suture and cinch. 22 In contrast, 1 X-Tack suturing system along with a cinch costs up to $695. Determining best practice and cost-efficiency of these various closure devices is desirable.…”

“…Various through the scope endoscopic clips have allowed for control of gastrointestinal bleeding, and approximation of mucosal injuries and full thickness defects ( Figure 1 , Video 1 : 0:09, 1:22). The use of endoscopic clips was initially described over 30 years ago ( 1 ). Since then, the variety and functionality of modern-day clips has progressed exponentially.…”

“…Most of the modern through the scope clips can be used in an endoscope with a 2.8-mm working channel, which is the majority of the diagnostic endoscopes on the market. These clips can be used as an effective monotherapy for hemostasis, when compared to injections or thermal coagulation ( 1 , 2 ). Overall, through the scope clips allow for excellent field exposure and visualization due to their compact size, and can be used in most commercially available endoscopes.…”
