Endoscopic Endonasal Transsphenoidal Approach to Pituitary Adenoma: A Multi-disciplinary Approach

Abstract: Background:The excellent visualization and minimally invasive approach employed in endoscopic endonasal procedures has now revolutionized the pituitary surgery, replacing the transnasal microscopic technique worldwide. However, it involves major shift in hand-eye co-ordination from static 3 dimensional images of microscope to 2 dimensional endoscopic images hence demands training and inter-disciplinary approach. Here we present our experiences in learning and developing a safe endonasal transsphenoidal endosco… Show more

“…Endoscopic visualization provides a more panoramic view of the operative field, compared with the microscope, allowing for better viewing of the suprasellar region ( 51 , 52 ). It is also possible to use instruments with a variety of angles to access lateral invasions of tumors ( 12 , 13 ).…”

“…For example, Gardner et al performed balloon occlusion in a tumor surrounding the ICA (18). If the test occlusion fails in the patient, care should be taken not to injure the ICA during surgery (19). The limitation of the case is no stent available at that time where an endovascular balloon was applied to occlude the left ICA.…”

“…The two-hand technique, utilizing a one-and-a-half nostril approach, offers more refined manipulation. Notably, the two-hand/one-and-a-half nostril technique in single-surgeon endoscopic endonasal transsphenoidal surgery has been shown to be safe and effective for excising large pituitary tumors [80,81].…”
