Endoscopic Management of Difficult Bile Duct Stones

Abstract: Endoscopic papillary large balloon dilation with balloon extraction ± mechanical lithotripsy is the initial approach used to retrieve large bile duct stones. With advent of digital cholangioscopy, electrohydraulic and laser lithotripsy are gaining popularity. Enteroscopy-assisted or laparoscopic-assisted approaches can be used for those with gastric bypass anatomy. Difficulties in removing bile duct stones can be related to stone-related factors such as the size and location of the stone or to altered anatomy … Show more

“…Mechanical lithotripsy was once the preferred method to facilitate stone fragmentation and extraction of large CBD stones [14]. However, in many experienced centers, mechanical lithotripsy is now falling behind EPLBD [15]. Digital cholangioscopyguided laser lithotripsy or electrohydraulic lithotripsy have become more popular owing to the convenience in assembly [14,16,17].…”

“…Lithotripsy can be performed using a mechanical lithotripsy (ML) device or cholangioscopy-directed lithotripsy. 12 ML is performed using a lithotripsy compatible basket to crush large stones in small fragments for extraction. The basket is attached to a lithotripsy handle and crank that is rotated until the stone is crushed by the lithotripsy wire and cable.…”

“…Since its introduction in 1974, ERCP has become the standard method for removing bile duct stones, which involves endoscopic sphincterotomy with balloon or basket for removal of stones. The success rate for removal of common bile duct stones ranges from 80 to 90% [ 3 , 4 ]. An unusual but possible complication is Dormia basket impaction or Traction cable breakage when removing CBD stones.…”
