Endoscopic Resection of Localized Pigmented Villonodular Synovitis in the Hoffa Fat Pad

Abstract: Pathologies within the fat pad can have either intrinsic or extrinsic causes. Most of them are the consequences of trauma and degeneration, but inflammatory and neoplastic diseases can arise in the fat pad. Pigmented villonodular synovitis is the most common space-occupying lesion of the Hoffa fat pad, followed by ganglion. The purpose of this Technical Note is to describe the details of endoscopic resection of localized pigmented villonodular synovitis in the Hoffa fat pad. Hopefully, this can allow complete … Show more

“…If knee arthroscopy, patellar tendoscopy, or endoscopy of the Hoffa fat pad is needed, the standard anterolateral and anteromedial portals can be used for the bursoscopy, arthroscopy, tendoscopy, and endoscopy. [15][16][17] We consider septic bursitis as a relative contraindication of the endoscopic bursectomy. If one wants to perform endoscopic resection of an infected prepatellar bursa, precautions should be taken to prevent spreading of bacteria, such as avoidance of use of an Arthropump, adequate antibiotic cover, and insertion of a suction drain.…”
