Endoscopic thoracic microdiscectomy

Abstract: Object The purpose of this investigation was to evaluate surgical and neurological outcomes in thoracic disc surgery in a prospective fashion. Methods Quantifiable outcome data such as operating time, blood loss, duration of chest tube drainage, narcotic drug use, length of hospital stay (LOS), and long-term follow up of neurological function and pain-related symptoms were collected prospectively. “…Similar to the result reported by Oskunian [14] the incidence of worsening was 1.8%. Compared with Stillermann's and Fessler's revision of the literature [10,21], the percentage of worsening in these thoracoscopic series is the lowest reported in recent publications.…”

“…This result contrasts with our previous published data including our early results [19,20] with an overall complication rate of 23%. The former included redo surgeries, multilevel and instrumented cases, requiring a higher level of surgical skills and experience but also bearing the risk of a higher complication rate, these data are similar to those published by Oskunian [14] 24% and Anand [2] 21%, both with a similar inhomogeneous patient population.…”

“…Recently, minimally invasive thoracoscopic approaches have been developed for the surgical treatment of thoracic disc herniations and traumatic fractures to overcome the problems associated with thoracotomy. 1,3,4,10,18,29,30,37 We here describe the application of a minimally invasive procedure for the surgical treatment of metastatic spinal cord compression and pathological fractures in the thoracolumbar spine, with an emphasis on recently developed reconstruction and stabilization techniques. Object.…”

“…The incidence is estimated to be between 0.25 and 1% of all such lesions, and population studies indicate that the overall incidence of thoracic disc herniations is approximately 1 per million patient-years [1,2]. Thoracic disc herniations typically present with a variety of nonspecific symptoms, frequently leading to a wrong or delayed diagnosis [2][3][4]. A variety of surgical approaches have been described to reach these anatomically challenging lesions including open transthoracic, costotransversectomy or lateral extracavitary approach, transpedicular or transfacet approaches, as well as thoracoscopic microdiscectomy [2,3,[5][6][7].…”
