Endoscopic treatment of pancreaticopleural fistulas

Abstract: IntroductionPancreaticopleural fistula (PPF) is a serious complication of acute and chronic pancreatitis.ObjectiveTo evaluate the effectiveness of various endoscopic techniques for the treatment of patients with PPFs.MethodologyProspective analysis of the results of endoscopic treatment of 22 patients with PPF due to pancreatitis was conducted at the Department of General, Gastroenterological, and Oncological Surgery, Ludwik Rydygier Collegium Medicum in Bydgoszcz, Nicolaus Copernicus University in Torun, betw… Show more

“…12 ERCP confirms the diagnosis of PPF in 80% of cases and reveals a fistulous pathway in approximately 59%. 12,24,25 In the largest study on endotherapy of postinflammatory PPFs, 22 patients were treated endoscopically, 26,27 and technical success was achieved in all cases. Clinical success was achieved in 21 (95.45%), and long-term success of endoscopic treatment was noted in 19 (86.36%) patients.…”

“…Endoscopic management can be complicated by proximal strictures or gallstones preventing stent insertion, or disruptions too distal to reach [ 5 , 17 ]. J agielski et al [ 18 ] conducted a prospective study of 22 patients undergoing endoscopic management of PPF and successful closure of the fistula was observed in 21 out of 22 patients (95.45%). One patient had significant pancreatic duct disruption leading to ongoing endoscopic drainage, and the PPF reoccurred in two patients during the short follow-up period of 1 year.…”
