Endoscopic-ultrasound evaluation and fine needle aspiration with a linear echoendoscope in the cecum: it is possible

Abstract: The combined overtube-guidewire technique using a linear-array echoendoscope for endoscopic ultrasound-guided evaluation and fine needle aspiration of a cecal subepithelial lesion (Case #1) and for staging of an early neoplastic lesion (Case #2).

“…Although an EUS-FNAB with a forward-viewing echoendoscope might be able to overcome scope intubation issues ( 11 ), it would be difficult to pinpoint the target lesion with the narrow scanning range (90°) of such an approach ( 3 ). The overtube and guidewire techniques ( 12 , 13 ) assist with scope intubation, although they are costly. We performed ultra-stiff guidewire-assisted scope intubation using a shallow oblique view (40°) echoendoscope under fluoroscopic and endoscopic guidance ( 3 ).…”

“…An endoscopic ultrasound-guided fine-needle aspiration biopsy (EUS-FNAB) is a well-established tissue sampling method (1), although it remains challenging to perform using a linear echoendoscope in the proximal colon because of its depictability and scope of intubation (2)(3)(4).…”

“…The endoscopist can leave a guidewire for extra assistance. Then they can insert the echoendoscope to conduct the EUS examination with or without tissue acquisition[ 24 , 25 ]. This technique was deployed and reported by Cheng et al [ 25 ].…”

“…2,3 In 2019, we reported a case of successful fine-needle aspiration (FNA) of a subepithelial lesion in the cecum using a CLA-EUS. 4 The potential use of CLA-EUS in the colon beyond the rectosigmoid area is significant because it can allow tissue sampling and enable EUS-guided interventions in the lower gastrointestinal (LGI) tract. Nonetheless, a standardized technique must be developed to ensure the safety and reproducibility of the procedure.…”
